# Avoid antiandrogen monotherapy unless adequate contraception is in place

## STEP 1 — Gather Information
Document patient-important hirsutism despite cosmetic measures, assess menstrual pattern, sexual activity, current contraception method, fertility desire, history of sterilization, and comorbidities (e.g., VTE risk). → Proceed to step 2.

## STEP 2 — Rule In / Rule Out
Is adequate contraception in place? (Defined as permanent sterilization, long-acting reversible contraception, or consistent use of a reliable contraceptive method). If NO → rule out antiandrogen monotherapy; move to alternative initial therapy. If YES → proceed to step 3.

## STEP 3 — Classify or Stratify
Among those with adequate contraception, classify patient preference regarding efficacy, side effects, and cost for OC vs antiandrogen. If patient prefers OC → proceed to OC prescription; if prefers antiandrogen → proceed to antiandrogen monotherapy consideration.

## STEP 4 — Decide
Prescribe the chosen therapy (OC or antiandrogen monotherapy) after confirming adequate contraception; if contraception inadequate, prescribe OC or direct hair removal based on hirsutism severity and patient preference, but avoid antiandrogen monotherapy.

## Clinical Guardrails / Mimics / Pitfalls
Do not prescribe antiandrogen monotherapy without confirming reliable contraception; be aware of teratogenic risk; avoid in pregnancy or if pregnancy possible; monitor liver function if using flutamide (though not recommended); consider VTE risk when selecting OC formulation.

## Concrete Clinical Example
A 24-year-old woman with patient-important hirsutism despite shaving, sexually active, using condoms inconsistently, desires pregnancy in future. Adequate contraception? No. Therefore, antiandrogen monotherapy contraindicated; start combined estrogen-progestin OC as initial therapy.
