# Measure lipid profile before and after treatment of growth hormone excess in acromegaly

## STEP 1 — Gather Information
Confirm acromegaly diagnosis (elevated IGF-1, nonsuppressed GH on OGTT) and plan/initiation of GH-excess directed therapy (surgery, medical, radiation).  
Action: Proceed to lipid assessment.

## STEP 2 — Rule In / Rule Out
Rule in: Patient is treatment-naïve for GH excess (no prior surgery, meds, radiation). Rule out: Patient already on GH-excess therapy.  
If treatment-naïve → obtain baseline lipid panel; if already on therapy → assess timing of last lipid measurement.

## STEP 3 — Classify or Stratify
Classify lipid profile: normal (LDL-C <100 mg/dL, HDL-C >40 mg/dL men/>50 mg/dL women, TG <150 mg/dL) vs abnormal (any outside thresholds).  
Action: If abnormal, consider lipid-lowering therapy per primary guideline; if normal, note baseline.

## STEP 4 — Decide
After GH-excess treatment and biochemical control (IGF-1 normalized, GH suppressed), repeat lipid panel.  
Action: Compare to baseline; if significant change (e.g., TG decrease >20% or LDL-C change), document and consider adjusting lipid-lowering therapy.

## Clinical Guardrails / Mimics / Pitfalls
Do not rely on GH replacement alone to lower LDL-C; do not ignore lipid changes if visceral obesity or diabetes develops; ensure IGF-1 confirms biochemical control before post-treatment lipid measurement; avoid measuring lipids during acute illness or uncontrolled hyperglycemia.

## Concrete Clinical Example
A 45-year-old with newly diagnosed acromegaly (IGF-1 2× ULN, nonsuppressed GH) scheduled for transsphenoidal adenomectomy. Baseline lipid panel shows TG 180 mg/dL, LDL-C 130 mg/dL, HDL-C 38 mg/dL. Patient starts octreotide LAR post‑surgery. Six months later, IGF-1 normalized, repeat lipid panel shows TG 110 mg/dL, LDL-C 115 mg/dL, HDL-C 42 mg/dL. Improvement in TG noted; continue monitoring.
