# Endo Paget Remission Target Turnover

> Suggests reducing bone turnover below the midpoint of the reference range for the chosen monitoring bone turnover marker to maximize the duration of remission. Triggered when aiming to sustain long-term remission after treatment and the marker exceeds the midpoint.

# Target bone turnover below midpoint to maximize remission duration

## STEP 1 — Gather Information
Collect a bone turnover marker (serum total ALP, P1NP, BSAP, CTx, or NTx) and its laboratory reference range; calculate the midpoint of the reference range.

## STEP 2 — Rule In / Rule Out
If the post‑treatment marker value is above the midpoint, rule in the need for further suppression; if at or below the midpoint, rule out (target achieved).

## STEP 3 — Classify or Stratify
Stratify the degree of excess above midpoint: mild (1‑25% above), moderate (26‑50% above), marked (>50% above) to guide intervention intensity.

## STEP 4 — Decide
For mild excess, optimize current regimen and repeat marker in 3‑6 months; for moderate or marked excess, consider a repeat potent bisphosphonate (e.g., second zoledronate infusion) or switch to a more potent agent, then re‑evaluate the marker.

## Clinical Guardrails / Mimics / Pitfalls
Do not rely on symptoms alone; avoid over‑suppression that may cause adynamic bone; ensure adequate renal function before potent bisphosphonate; use least significant change to distinguish true change from assay variability; confirm marker specificity (e.g., bone‑specific ALP if liver disease); do not mistake transient fluctuations for true biological change.

## Concrete Clinical Example
A 68‑year‑old with polyostotic Paget’s disease received a single 5 mg IV zoledronate. Six months later serum ALP was 78 U/L (reference 30‑120 U/L, midpoint 75 U/L). Because the value was just above the midpoint, a second zoledronate infusion was given. At the following 6‑month visit ALP had fallen to 42 U/L (below midpoint), and the patient remained in biochemical remission at 5‑year follow‑up.
