# Es Cushing Discontinue Glucocorticoid Normal Hpa Response

> This skill recommends discontinuing glucocorticoid when the response to HPA axis recovery tests (morning cortisol, ACTH stimulation, or insulin-induced hypoglycemia) is normal, indicating adequate adrenal function recovery. Use in postoperative Cushing’s syndrome patients with at least one intact adrenal gland when test results are available.

# Discontinuing Glucocorticoid When HPA Axis Recovery Response Is Normal

## STEP 1 — Gather Information
Confirm postoperative Cushing’s syndrome patient with at least one intact adrenal gland. Obtain morning serum cortisol, ACTH stimulation test (baseline and peak cortisol), or insulin-induced hypoglycemia test results.

## STEP 2 — Rule In / Rule Out
Determine if HPA axis recovery is normal: morning cortisol ≥5 µg/dL (138 nmol/L) OR ACTH-stimulated cortisol ≥18 µg/dL (500 nmol/L) OR adequate cortisol rise during insulin-induced hypoglycemia (per local lab norms). If any test meets normal criteria, proceed; otherwise, HPA axis not recovered.

## STEP 3 — Classify or Stratify
If normal response: classify as HPA axis recovered. If subnormal: classify as HPA axis insufficient, requiring continued glucocorticoid replacement.

## STEP 4 — Decide
Discontinue glucocorticoid (e.g., taper hydrocortisone off) and provide patient education on adrenal insufficiency signs and stress dosing. If not normal, continue glucocorticoid and schedule repeat testing in 3–6 months.

## Clinical Guardrails / Mimics / Pitfalls
Do not discontinue glucocorticoid if any test is subnormal; monitor for glucocorticoid withdrawal symptoms (fatigue, nausea, weight loss). Ensure patient knows stress-dosing for illness. Avoid mistaking withdrawal for recurrence; assess clinically and biochemically if symptoms persist.

## Concrete Clinical Example
A 45‑year‑old woman 8 months after transsphenoidal resection of an ACTH‑secreting pituitary adenoma has morning cortisol 6.2 µg/dL and ACTH stimulation peak 22 µg/dL. Both exceed normal thresholds, so glucocorticoid is tapered and discontinued with instructions on stress dosing.
