# Limit non-academic screen time for pediatric obesity prevention

## STEP 1 — Gather Information
Collect child's age, current daily non-academic screen time (hours), type of screen use (gaming, video, social media), academic screen use, and physical activity level; document any comorbid conditions.
**Action:** Record screen time and compare to the 2‑hour threshold.

## STEP 2 — Rule In / Rule Out
Is non‑academic screen time >2 hours per day?
- **Yes:** Proceed to step 3.
- **No:** Reinforce current limits and encourage maintenance of healthy habits.
**Decision:** Determine need for intervention based on threshold.

## STEP 3 — Classify or Stratify
Classify screen time into:
- **<1 hour/day** (low)
- **1–2 hours/day** (borderline)
- **>2 hours/day** (excessive)
Also note proportion of sedentary non‑screen activities (e.g., reading, sitting).
**Action:** Assign category and plan targeted counseling.

## STEP 4 — Decide
- If **excessive (>2 hr):** Set goal to reduce to ≤2 hr/day, replace displaced time with ≥60 min moderate‑vigorous physical activity, and counsel on alternative non‑sedentary hobbies.
- If **borderline (1–2 hr):** Encourage maintaining ≤2 hr, add 30 min extra physical activity, and monitor for creep.
- If **low (<1 hr):** Reinforce current habits, promote continued physical activity, and reassess at next visit.
**Action:** Provide specific screen‑time reduction plan and activity prescription.

## Clinical Guardrails / Mimics / Pitfalls
Do not count educational screen use toward the limit; avoid assuming parental reports are accurate without corroboration; do not recommend complete screen elimination as it may be unrealistic and counterproductive; remember that other sedentary behaviors (e.g., prolonged sitting, reading) also contribute to risk; ensure physical activity recommendations are age‑appropriate and safe.

## Concrete Clinical Example
A 10‑year‑old with BMI at the 88th percentile reports 3 hours of video games and 1 hour of TV daily (total 4 hr non‑academic screen time) and gets <20 min of active play. Counsel family to limit recreational screen to 1 hr/day, replace 2 hr with outdoor play or sports, and set a screen‑free bedroom rule; re‑evaluate in 1 month.
