# Eso Pediatric Obesity Total Cholesterol Interpretation

> This skill interprets fasting total cholesterol levels to screen for dyslipidemia in children and adolescents undergoing obesity evaluation. Triggered by pediatric obesity screening (BMI ≥85th percentile) or dyslipidemia evaluation, it classifies cholesterol as acceptable (<170 mg/dL), borderline high (170‑199 mg/dL), or high (≥200 mg/dL) per Endocrine Society guidelines.

# Interpret total cholesterol for dyslipidemia screening

## STEP 1 — Gather Information
Obtain a fasting total cholesterol measurement, patient age, and confirm the child/adolescent is being evaluated for obesity (BMI ≥85th percentile) or dyslipidemia risk.

## STEP 2 — Rule In / Rule Out
Is the fasting total cholesterol <170 mg/dL? If yes, classify as acceptable and rule out dyslipidemia; if ≥170 mg/dL, proceed to further classification.

## STEP 3 — Classify or Stratify
If total cholesterol is 170‑199 mg/dL, classify as borderline high; if ≥200 mg/dL, classify as high.

## STEP 4 — Decide
For acceptable: continue routine obesity screening per guideline. For borderline high: repeat fasting lipid panel in 3 months and initiate lifestyle counseling. For high: obtain full fasting lipid panel, intensify lifestyle intervention, and consider referral for pharmacotherapy per pediatric dyslipidemia guidelines.

## Clinical Guardrails / Mimics / Pitfalls
Ensure the sample is fasting; non‑fasting specimens may overestimate cholesterol. Do not diagnose dyslipidemia on a single non‑fasting result. Consider acute illness, medications, or recent dietary changes that can transiently elevate lipids. Do not ignore concomitant triglyceride or LDL abnormalities.

## Concrete Clinical Example
A 12‑year‑old with BMI at the 98th percentile has a fasting total cholesterol of 185 mg/dL. This falls in the borderline high range; the clinician orders a repeat fasting lipid panel in 3 months and provides lifestyle counseling on diet and activity.
