# Optimize blood sampling conditions for PA screening

## STEP 1 — Gather Information
- Record time of day, fasting status (≥8 h overnight fast), and patient posture (supine vs sitting) at blood draw.
- Note any recent food intake, caffeine, nicotine, or vigorous exercise that could influence aldosterone or renin.
- Document whether the sample was obtained in the sitting position at any time.

## STEP 2 — Rule In / Rule Out
- Is the sample obtained in the **sitting position** (regardless of fasting or time of day)?  
  - **Yes** → proceed to Step 3 (acceptable per guideline).  
  - **No** → proceed to Step 3 (non‑ideal supine sample).

## STEP 3 — Classify or Stratify
- **Sitting sample**: classify as **acceptable** for screening; no further stratification needed.  
- **Supine sample**: classify as **non‑ideal**; assess fasting status and time of day:  
  - If fasting and early morning → ideal (proceed with standard interpretation).  
  - If non‑fasting or not early morning → **suboptimal**; flag for potential reduced sensitivity/specificity.

## STEP 4 — Decide
- For **acceptable** (sitting) or **ideal** supine samples: apply standard screening cutoffs (ARR ≥ 200 or PAC/ARC ≥ 40 with PAC ≥ 60 pg/mL).  
- For **suboptimal** supine samples: either repeat the draw under ideal conditions if clinically feasible, or proceed with screening while interpreting results with caution (consider borderline ARR/PAC values as provisionally positive and pursue individualized management).

## Clinical Guardrails / Mimics / Pitfalls
- Do **not** rely solely on a non‑ideal supine sample to exclude PA; false‑negative risk increases.  
- Avoid assuming sitting position eliminates all preanalytical influences; still control for recent meals, stress, and medications that affect renin.  
- Do **not** ignore the preference for early‑morning supine fasting when it is readily obtainable; use it to maximize test accuracy.

## Concrete Clinical Example
A 58‑year‑old hypertensive patient has blood drawn in the sitting position after a light breakfast. The clinician questions validity. According to the guideline, sitting sampling is acceptable; the sample is processed with ARR ≥ 200 and PAC ≥ 60 pg/mL cutoffs, and screening proceeds without repetition.
