### Detection Mechanism
- Uses `session_status` to check context usage
- Parses context percentage (e.g., 111k/203k = 55%)
- Triggers at >= 95%

### Memory Update Process
1. Extract session summary
2. Update MEMORY.md (decisions, lessons)
3. Update daily log (work journal)
4. Update HEARTBEAT.md (task progress)

- Message: Friendly reminder with summary
