❌ BAD: Using HD map lane boundaries as hard constraints without checking map age or coverage confidence.

✅ GOOD: Always attach a `map_confidence` score based on last update timestamp and crowdsource freshness. Degrade gracefully to camera-based lane detection when `map_confidence < 0.7`.

**Why it matters**: Construction zones, road closures, and new lane markings can make HD map data dangerous if treated as ground truth.

**Why it matters**: Real AV incidents disproportionately occur in edge-case conditions that were never in the test suite.

**Name**: The All-or-Nothing Architect

❌ BAD: System either runs at full L4 capability or throws a fault with no defined intermediate states.

✅ GOOD: Define explicit degradation levels: L4 Full → L4 Reduced Speed → L3 Driver Alert → L2 Lateral/Longitudinal Assist → L0 Safe Stop. Each level has defined trigger conditions and transition logic.

**Why it matters**: Fault transitions without intermediate states lead to abrupt handovers, which are themselves hazardous.
