# Incident Root Cause Analysis (Hybrid)

Produce an **executive-ready, evidence-based RCA** suitable for incident review, engineering leadership,
and postmortem documentation. Separate facts, hypothesis confidence, evidence gaps, and next steps.
**Correlation may increase confidence but never establishes causation without independent supporting
evidence** — rank hypotheses; never assert a single cause when evidence is thin.

## Required inputs

Parse per [workflow/inputs.md](workflow/inputs.md). **Ask before Phase 0** if missing — do not invent.

**Time window** (required unless `jira_key` anchors it):

- `from_time` / `to_time` with timezone confirmed

**At least one anchor:**

**Unknown policy:** if no hypothesis exceeds **MEDIUM** after caps → conclude *No defensible root cause*;

- **Never invent utilization, error rates, or log lines** — use `missing` / `unknown`; cite attempted queries in **Gaps**

At least one observability capability is required; Datadog and KubeSense are provider examples, not
registered skills. Optional: GitLab, Jenkins, Jira, correlator CLI ([SETUP.md](SETUP.md)).

## Hard stops (halt the phase)

- Phase 4 with empty `error_signals` **and** `infra_signals` → blocked report, no ranking
- **Circular causal graph** → acyclic graph required; feedback loops in prose only
- **Best-guess primary** when all hypotheses ≤ MEDIUM → *No defensible root cause*

## Confidence caps and corrections (continue, downgrade or fix)

- **HIGH** with one observability source → cap **MEDIUM**
- Assert cause at **LOW/UNKNOWN** → hypothesis + gaps
- CLI absent but claim CLI ranking → manual scoring + Gaps note
- **Infrastructure symptom alone** as root cause without trigger + systemic layer — [root-cause-depth.md](reference/root-cause-depth.md)
- **KubeSense ✅ but skipped** when log fallback triggered → `mcp_process_failure`
- **Split one causal chain into multiple hypotheses** — merge into graph
- **Conflicting evidence ignored** → explain in Gaps; unresolved → max **MEDIUM**
