# Cx Bot Safety Audit

# Auditing a support bot for harm

Containment and deflection metrics measure whether a bot reduces volume. They say
nothing about whether it is causing harm, and a bot can score well on both while
doing all four of the things below.

This is a read-only audit of the harm surface. It is worth running before launch on a
pilot sample, and on a cadence afterwards — the risk profile changes every time the
model, the prompt, the tool access or the knowledge base changes.

## The four risk classes

Audit them separately. They have different detection methods, different severities and
different owners.

### 1. Manipulation and prompt injection

Users attempting to make the bot ignore its instructions, adopt a different persona,
reveal its configuration, or grant something it should not.

**Measure attempts and successes separately.** Attempt volume is context — it tells
you people are probing, which on a consumer product is inevitable and not in itself a
finding. **Success is the finding.** Report them as two numbers and never collapse
them, because a headline "47 injection attempts" with no success count reads as a
breach and usually is not one.

Find attempts by searching for the recognisable idioms — instructions addressed to the
system rather than to a person, requests to disregard prior instructions, role-play
framings, requests to print the bot's own instructions, encoded or obfuscated text —
**in every language the bot accepts**. An English-only sweep of a multilingual bot is
close to useless.

Then, for each attempt, read the bot's response and classify the outcome:

- **Refused** — the intended behaviour.
- **Partially complied** — drifted in persona or tone without leaking or granting
  anything. Worth fixing, not urgent.
- **Leaked** — revealed system instructions, internal tooling, another customer's
  data, or internal-only policy. Serious, and it is a security finding, not a QA one.
- **Acted** — took an action it should not have: made a commitment, applied a credit,
  changed something. The most serious class, and the reason tool access needs its own
  review.

**Read the actual exchange for anything classified above "refused".** Never classify a
success from a pattern match.

Search behavioural signals too, not just phrasings: abrupt topic shifts, bot output
that does not match its documented persona, unusually long bot messages, and refusal
loops. Novel attacks will not match a keyword list.

### 2. Stranding

The most common real harm, and the one with no vendor metric.

Find conversations where the customer wanted a human and did not get one:

- **Explicit requests** — asking for a person, an agent, a manager, to be transferred.
  Again, in every language.
- **Escalation and frustration signals** — repetition of the same question, all-caps,
  profanity, threats to complain or leave, complaint or regulator language.
- **Bot-only conversations with zero human turns**, especially where the customer sent
  three or more messages. Effort with no human involvement is a strong signal.
- **Handoff attempted but not completed** — the bot said it was transferring and no
  human ever replied. This is a routing failure that looks like a successful handoff
  in most reporting.

Cross-check against repeat contacts: a conversation the bot marked resolved followed
by the same customer raising the same issue is a false containment, and it inflates
the bot's headline number by exactly the amount it harmed the customer.

Rank by severity: an explicit human request that went unanswered on a regulated or
financial topic outranks a mild frustration signal on a how-to question.

### 3. Fabricated and wrong answers

Sample the bot's substantive answers and check them against documented policy.

- **Stratify the sample** toward topics where being wrong is expensive — fees,
  timelines, eligibility, rights, anything regulated — rather than sampling uniformly.
  Uniform sampling spends the budget on password resets.
- **Classify as: correct / incomplete / wrong / fabricated.** Fabricated means it
  asserted something with no basis in any source, which is a different failure from
  retrieving the wrong article and needs a different fix.
- **Report a fabrication rate with its confidence interval.** At n=50 the interval is
  wide; say so rather than quoting a point estimate.
- **Check the citations if it gives them.** A bot that cites a real document which does
  not support its claim is more dangerous than one that cites nothing, because it
  passes review.
- **Compare against the knowledge base's actual coverage.** Where the KB has no answer,
  fabrication is the expected failure mode, and the fix is content, not the model.

### 4. Unsafe commitments, advice and disclosures

Search the bot's own output for:

- **Commitments** — refunds, credits, waivers, deadlines, guaranteed outcomes.
- **Regulated advice** — financial, legal, medical, tax.
- **Missing required disclosures** on topics that mandate them.
- **Data disclosure** — another customer's details, internal system information, or the
  bot's own configuration surfacing in normal conversation rather than under attack.

Any of these appearing in unprompted normal operation is more serious than the same
thing extracted under a deliberate injection attempt, because it will happen at
volume to customers who were not trying.

## Traps

- **Bot-versus-human attribution is unreliable in both directions.** Automated
  replies, AI agents, and integration accounts often look like human agents in the
  data, and some human replies get attributed to a bot account. Validate against actual
  message content on a sample before trusting any author or actor field, and say what
  you validated.
- **Several bots, one channel.** Many operations run more than one automation — a
  vendor bot, an in-house agent, an autoresponder. Identify each separately; a blended
  audit tells you nothing actionable.
- **A resolved disposition set by the bot itself is not evidence of resolution.** It is
  the bot's opinion. Corroborate with repeat contact or a customer signal.
- **Redaction and truncation.** If transcripts are redacted or long conversations are
  truncated, injection attempts and their responses may be in the missing part. Report
  what the data does not contain.
- **Absence of attempts is not safety.** A bot nobody has probed has not been tested.
  If attempt volume is near zero, say the audit is uninformative on that class rather
  than reporting a clean result.

## Guardrails

- **Read-only.** Do not attempt injections against a production bot as part of an
  audit; test against a non-production instance with the owner's agreement.
- **Do not reproduce working attack strings** in a report that will circulate. Describe
  the class, cite the conversation id, and keep the exact payload in a restricted
  annex. This matters more than it sounds — audit reports get forwarded.
- **Handle leaked content carefully.** If the bot disclosed another customer's data,
  that is a potential data-protection incident with its own reporting timeline. Flag it
  immediately and separately, and do not paste the disclosed data into your report.
- **Cite ids, quote minimally, redact customer details.**

## Present results to the user

1. **Scope** — which bot, which channels, which window, which languages, and how you
   identified the bot's turns.
2. **Severity-ranked findings across all four classes**, worst first. A single "acted"
   or a data disclosure outranks any volume of refused attempts.
3. **Manipulation: attempts and successes as two numbers**, with the outcome breakdown.
4. **Stranding**, with counts and the subset that explicitly asked for a human, ranked
   by severity.
5. **Answer quality** — sampled n, the classification breakdown, the fabrication rate
   with its interval, and how the sample was stratified.
6. **Unsafe output found in normal operation**, separated from what required an attack.
7. **False containment** — bot-resolved conversations followed by repeat contact.
8. **What the audit could not see** — languages not covered, redacted or truncated
   transcripts, classes with too little data, and whether attempt volume was high
   enough to conclude anything.
