Separate:

- incident observations
- architectural facts
- hypotheses
- verified root cause

## Build Expected Flow

Use current architecture to reconstruct the expected path for the affected operation.

## Timeline

## Hypotheses

Do not label root cause VERIFIED unless direct evidence supports it.
