2. **Remediation Requires Explicit Approval**: Any write, cancellation, or restart must go through `approval_broker.py` plan-and-commit.
3. **Audit Trail**: Every incident step, diagnostic hypothesis, and resolution outcome is logged in `MEMORY.md`.
