# Diabetes Variance — Insulin-Dependent Drivers

Insulin-dependent diabetic drivers can be certified to operate CMVs under the 2018 Final Rule — but only after meeting strict criteria.

## Eligibility
A driver with insulin-treated diabetes mellitus may be certified if:
- Stable insulin treatment for at least 2 months
- Demonstrated ability to monitor blood glucose
- No severe hypoglycemic events in past 6 months
- Hemoglobin A1c < 10%
- Annual medical examination
- Quarterly check-ins with treating physician

## Self-monitoring requirements
Driver must monitor blood glucose:
- Within 1 hour BEFORE driving
- Every 2-4 hours WHILE driving
- After driving completes
- Record in glucose log

Glucose target during driving: 100-300 mg/dL

## Documentation provided to medical examiner
- Treating clinician's report (every 12 months)
- Glucose monitoring logs (recent 30-90 days)
- Insulin prescription and dosing history
- Severe hypoglycemic event report (if any)
- HbA1c results

## Continued certification considerations
Annual medical exam OR more frequent based on examiner judgment. The examiner has authority to set shorter intervals (e.g., every 6 months for newer insulin users).

## What disqualifies a driver
- Severe hypoglycemic event in past 6 months
- HbA1c ≥ 10%
- Inability to self-monitor reliably
- Documented end-organ damage (kidney, retinopathy, neuropathy) at level that affects driving

## Common pitfall
Driver moves from oral diabetes medication to insulin during cert period. The driver must notify the carrier; the carrier must require new medical exam before continuing safety-sensitive duty (the prior cert was issued for oral, not insulin).
