Load when universal-skill-creator finishes creating a skill, split-skill extracts a child, deprecate-skill retires a skill, improve-skills makes a structural change, or the user manually renames or restructures skills. Also triggers on "update the skill index", "sync skill references", "refresh the skill graph", "fix broken skill cross-references", or "update docs after skill change".
