Five Whys Root Cause Analysis Skill

This skill applies the Five Whys methodology to systematically drill down from symptoms to root causes, enabling permanent solutions rather than temporary fixes.

Theoretical Foundation

Origin and Development

The Five Whys technique was developed by Sakichi Toyoda in the 1930s and became a core component of the Toyota Production System (TPS). It's now fundamental to Lean Manufacturing, Six Sigma, and Kaizen methodologies.

Core Principle

The fundamental insight is that most problems are symptoms, not causes. By repeatedly asking "Why?" (typically 5 times), we drill past symptoms to reach actionable root causes.

Key Principles

1. Ask "Why?" until the cause is within control - Stop when you reach something you can fix
2. Multiple root causes are normal - Problems often have several contributing causes
3. Focus on process, not people - "Why did the process allow this?" not "Who made this mistake?"
4. Verify each level - Each "because" should be factual, not assumed
5. Five is a guideline, not a rule - Could be 3 whys or 7; stop at the actionable root

Step 2: Problem Statement Definition

Capture the problem precisely:

Questions:

• "What specific problem are we investigating?"
• "When did this problem occur or start?"
• "How frequently does it happen?"
• "What is the impact?"
• "Where specifically does it occur?"

Step 3: First "Why" Analysis

• If multiple answers exist, document all (creates branches)
• Verify answer with data/evidence

Step 5: Root Cause Verification

Confirm we've reached the root:

Verification Tests:

1. Control Test: "Is this something we can fix/influence?"
2. Chain Test: "If we fix this, does it prevent the problem?"
3. Recurrence Test: "Will fixing this prevent future occurrences?"
4. Walk-Forward Test: Start from root cause, walk forward—does it lead to the original problem?

If tests fail: Continue asking "Why?" or branch to explore other causes.

Step 6: Handle Multiple Cause Chains

Real problems often have multiple contributing causes:
