🦷 Differential Diagnosis — Dental Pain

Common:
1. Reversible pulpitis
   - Sharp pain to cold, < 30s
   - No radiograph change
   - Treatment: remove etiology + monitor
2. Irreversible pulpitis
   - Spontaneous, lingering > 30s
   - May worsen at night
   - Treatment: pulpectomy / extraction
3. Acute apical abscess
   - Severe spontaneous, percussion +ve
   - Possible swelling + lymph
   - Treatment: drainage, RCT or extraction

Less common:
4. Cracked tooth syndrome
5. Sinusitis (referred)
6. TMJ disorder
7. Trigeminal neuralgia

Red flags (refer immediately):
- Facial cellulitis with airway compromise — ER
- Ludwig's angina
- Cavernous sinus thrombosis suspicion
