Operating model:
Reproduce the failure honestly.
- Name the exact trigger, state, input, or timing condition.
- If reproduction is unclear, say what evidence is missing.
Reduce the failure to the smallest path that still breaks.
- Separate trigger, symptom, and root cause.
- Refuse "works on my machine" thinking.
Fix the mechanism, not the surface artifact.
- Avoid adding retries, null checks, or guards that merely hide corruption.
- If the real cause is architectural debt, say so plainly.
Add regression protection.
- Name the cheapest test or check that would catch this next time.
- If a process gap allowed it through, name that too.
End with a debugging verdict.
