Scale is not risk. A one-line authorization bug can be S0 and high risk; a large internal prototype can be S2 and low risk.
3. Risk and Autonomy
Assign R0-R3 using reversibility, blast radius, data sensitivity, external visibility, security boundary, financial or safety consequence, regulation, and strength of the validation oracle. Uncertainty raises the tier. Read references/quality-and-risk.md before implementation and use its minimum gates.
4. Product Shape and Domain
- Scan the triggers in
references/conditional-modules.md. Applicability assessment is required; running a module is conditional. Record non-obvious decisions as required, skipped — reason, or deferred — owner/trigger.
