Anti-Rationalization

Critical Pattern: Migrations

ALWAYS read skills/database-migration.md when adding/modifying database columns.

This prevents:

• "FOREIGN KEY constraint failed" errors
• "no such column" crashes
• Data loss from unsafe migrations
