Determine whether the patient exhibits symptoms suggestive of gallstone disease. If symptoms are present, proceed to abdominal ultrasound; if symptoms are absent, skip routine ultrasound.
STEP 3 — Classify or Stratify
Classify the patient as asymptomatic (no gallstone-related symptoms) or symptomatic (presence of gallstone-related symptoms). This classification guides the need for imaging.
STEP 4 — Decide
Withhold routine abdominal ultrasound in asymptomatic patients receiving SRLs; obtain abdominal ultrasound only when new or worsening gallstone symptoms develop.

A 45-year-old woman with acromegaly on lanreotide autogel 90 mg monthly for 18 months reports no abdominal pain, nausea, or jaundice on routine follow-up. Physical exam is benign. According to the guideline, routine abdominal ultrasound is not indicated. She continues SRL therapy; if she later develops right upper quadrant pain, an abdominal ultrasound would then be ordered.
