STEP 1 — Gather Information
Confirm euthyroid state (TSH within laboratory reference range, free T4 normal) after treatment for hyperthyroidism (antithyroid medication, radioactive iodine, or thyroidectomy). Obtain a lipid panel (LDL-C, HDL-C, triglycerides, total cholesterol) and document the timing relative to achievement of euthyroidism.
Proceed to assess euthyroid status.
STEP 2 — Rule In / Rule Out
If TSH is suppressed (< lower limit) or elevated (> upper limit), or free T4 is abnormal, the patient is not euthyroid; continue managing thyroid status and recheck thyroid function in 4–6 weeks.
If TSH and free T4 are both within normal limits, the patient is euthyroid; proceed to lipid panel evaluation.
STEP 3 — Classify or Stratify
If the lipid panel was drawn <3 months after achieving euthyroid state, consider the results may not reflect steady-state lipid levels; schedule a repeat lipid panel at ≥3 months post-euthyroid.
If the lipid panel was drawn ≥3 months after euthyroid, interpret LDL-C changes as reflective of the euthyroid state.
STEP 4 — Decide
If LDL-C ≥70 mg/dL (or per the patient’s ASCVD risk threshold), initiate lifestyle modification and consider statin therapy according to lipid guidelines; if LDL-C <70 mg/dL and no other risk factors, reinforce healthy lifestyle and repeat lipid panel annually or as clinically indicated.
If LDL remains >100 mg/dL after lifestyle intervention, consider low-to-moderate intensity statin.
