STEP 1 — Gather Information

If the patient is on an SRL and reports new biliary symptoms, proceed to step 2.
STEP 2 — Rule In / Rule Out
Determine whether symptoms are typical of biliary colic/cholecystitis (e.g., postprandial RUQ pain >30 min, radiating to right scapula or shoulder, associated nausea/vomiting) versus atypical or non‑biliary causes. If typical features are present, rule in gallstone suspicion and go to step 3; otherwise, consider alternative diagnoses and do not order ultrasound at this time.
STEP 3 — Classify or Stratify
Assess for signs of complicated cholecystitis (fever >38 °C, leukocytosis, elevated CRP, positive Murphy's sign) versus uncomplicated biliary colic. If complicated cholecystitis is suspected, proceed to urgent ultrasound; if symptoms suggest uncomplicated colic, schedule a routine right upper quadrant ultrasound.
STEP 4 — Decide
Order an abdominal ultrasound focused on the gallbladder and biliary tree to evaluate for gallstones, sludge, wall thickening, pericholecystic fluid, or sonographic Murphy's sign.
Clinical Guardrails / Mimics / Pitfalls
Do not perform routine screening ultrasound in asymptomatic patients on SRL therapy; do not delay imaging when high suspicion for acute cholecystitis exists; consider mimics such as hepatitis, peptic ulcer disease, biliary dyskinesia, or renal colic; avoid relying on isolated abnormal LFTs without correlating symptoms; reserve CT or MRCP for equivocal US results when suspicion remains high.
Concrete Clinical Example
A 48‑year‑old woman on lanreotide autogel 120 mg monthly for acromegaly reports 45‑minute postprandial right upper quadrant pain with nausea twice over the past week; exam shows mild tenderness below the right costal margin, Murphy's sign equivocal, LFTs normal. Because she is on SRL and has typical biliary colic symptoms, a right upper quadrant ultrasound is ordered, revealing multiple gallstones without wall thickening; she is referred for surgical evaluation.
