Consider factors that may affect hemoglobin A1c when interpreting preoperative levels
STEP 1 — Gather Information
Collect patient's HbA1c, recent blood glucose values, history of anemia (e.g., iron deficiency, B12/folate deficiency, hemolysis), hemoglobinopathies (sickle cell, thalassemia), chronic renal failure (eGFR <60 mL/min/1.73m²), alcoholism, medications that affect HbA1c (e.g., opioids, high-dose vitamin C/E, antiretrovirals), and assess for large glucose variations (wide swings, frequent hypoglycemia/hyperglycemia).
STEP 2 — Rule In / Rule Out
Determine if any factor that could affect HbA1c reliability is present. If none, HbA1c can be used reliably; if any factor present, HbA1c may be unreliable.
STEP 3 — Classify or Stratify
If HbA1c deemed unreliable, classify patient for reliance on preoperative blood glucose targets; if reliable, proceed with HbA1c-based target.
STEP 4 — Decide
If HbA1c unreliable, target preoperative blood glucose 100–180 mg/dL (5.6–10 mmol/L) and consider repeating HbA1c after addressing factor or using alternative glycemic markers; if reliable, target preoperative HbA1c <8% (63.9 mmol/mol) and blood glucose 100–180 mg/dL.
Clinical Guardrails / Mimics / Pitfalls
Do not rely solely on HbA1c in patients with anemia, hemoglobinopathies, renal failure, alcoholism, or drug effects; do not ignore large glucose fluctuations that may mask chronic hyperglycemia; do not assume HbA1c reflects recent glycemic control when erythrocyte turnover is altered; avoid using HbA1c to deny surgery if unreliable without confirming with glucose metrics.
Concrete Clinical Example
A 68-year-old with type 2 diabetes and CKD stage 3b (eGFR 38) scheduled for elective hip replacement has HbA1c 7.6% but recent glucose logs show frequent hypoglycemia (<70 mg/dL) and hyperglycemia (>250 mg/dL). Given chronic renal failure and glucose variability, HbA1c is deemed unreliable; the team targets preoperative glucose 100–180 mg/dL and proceeds with surgery after achieving stable glucose in that range for 24 hours.
