Interpret fasting plasma glucose for prediabetes and diabetes screening

STEP 1 — Gather Information

Collect fasting plasma glucose result after ≥8 hours caloric fast; verify patient’s BMI percentile to confirm overweight/obese status (≥85th percentile) indicating need for comorbidity screening per Table 2.

STEP 2 — Rule In / Rule Out

Is fasting plasma glucose ≥126 mg/dL? If yes → rule in diabetes; if <100 mg/dL → rule out diabetes and prediabetes (normal); if 100–125 mg/dL → proceed to classify prediabetes.

STEP 3 — Classify or Stratify

For glucose 100–125 mg/dL: classify as impaired fasting glucose (prediabetes, IFG); for glucose <100 mg/dL: classify as normal.

STEP 4 — Decide

Normal: repeat screening annually per obesity guideline; Prediabetes: initiate lifestyle modification, repeat glucose in 3–6 months, monitor for progression; Diabetes: confirm with repeat testing if asymptomatic, then initiate diabetes management per ADA guidelines.

Clinical Guardrails / Mimics / Pitfalls

Do not use random glucose or HbA1c alone for diagnosis in pediatrics; ensure fasting state; avoid insulin concentrations; single abnormal result requires confirmation; consider acute illness, stress, or medication effects that may transiently elevate glucose.

Concrete Clinical Example

A 12‑year‑old with BMI at the 98th percentile has a fasting plasma glucose of 110 mg/dL after an overnight fast → classified as prediabetes (IFG) → lifestyle counseling and repeat testing in 4 weeks advised.
