STEP 1 — Gather Information
Collect patient age (≥2 years), BMI percentile (≥85th percentile indicates overweight/obesity), indication for OGTT (screening for dysglycemia per comorbidity evaluation), and ensure 8‑hour fast prior to administering 1.75 g/kg glucose (max 75 g). Record the 2‑hour plasma glucose result in mg/dL.
STEP 2 — Rule In / Rule Out
If the 2‑hour glucose is ≥200 mg/dL, rule in diabetes; otherwise proceed to evaluate for impaired glucose tolerance.
STEP 3 — Classify or Stratify
If the 2‑hour glucose is 140–199 mg/dL, classify as impaired glucose tolerance (IGT/prediabetes); if <140 mg/dL, classify as normal glucose tolerance.
STEP 4 — Decide
For normal glucose tolerance, continue routine screening per risk‑based schedule; for IGT, initiate intensive lifestyle modification and consider repeat OGTT in 6–12 months; for diabetes, confirm with repeat testing and initiate management per ADA pediatric diabetes guidelines.

Do not interpret OGTT results if the patient was not fasting, had acute illness, or received a non‑standard glucose load; avoid relying on fasting insulin or HbA1c alone for dysglycemia screening in pediatrics due to limited validity; remember that OGTT is preferred over HbA1c for detecting IGT in this age group.

A 12‑year‑old with BMI at the 96th percentile undergoes OGTT; 2‑hour glucose measures 165 mg/dL → classified as IGT → clinician prescribes diet/exercise plan and schedules repeat OGTT in 12 months to monitor progression.
