Interpret total cholesterol for dyslipidemia screening

STEP 1 — Gather Information

Obtain a fasting total cholesterol measurement, patient age, and confirm the child/adolescent is being evaluated for obesity (BMI ≥85th percentile) or dyslipidemia risk.

STEP 2 — Rule In / Rule Out

Is the fasting total cholesterol <170 mg/dL? If yes, classify as acceptable and rule out dyslipidemia; if ≥170 mg/dL, proceed to further classification.

STEP 3 — Classify or Stratify

If total cholesterol is 170‑199 mg/dL, classify as borderline high; if ≥200 mg/dL, classify as high.

For acceptable: continue routine obesity screening per guideline. For borderline high: repeat fasting lipid panel in 3 months and initiate lifestyle counseling.
