- Coupling issues: modules that are too tightly bound and will be affected by this feature
- Existing debt: TODOs,
anytypes, commented-out code, missing tests in areas this feature touches - Failure points: places where the new feature could break existing behavior (shared utilities, DB schemas, auth middleware)
- Test coverage gaps: files touched by this feature that have <80% test coverage (or no tests at all). Surface these as 🟠 HIGH severity — adding new code on top of untested code creates invisible regressions. Add a mandatory sub-task to the affected story: "Add missing tests for existing
<file>behavior before modifying it." If coverage tooling is not configured, note it as a USER-TASK. - Auth/permissions: does this feature add/modify routes, actions, or data access? If yes, add a security note to the PRD header listing affected routes and who can access them. Also add

Greenfield / new project: If there is no existing codebase (empty project or from scratch), skip the audit and note
## Codebase Health: Greenfield — no existing code to audit. Proceed directly to clarification. For greenfield projects, the clarification phase MUST establish the tech stack before any design work: language (TypeScript/Python/etc.), framework (Next.js/Remix/FastAPI/etc.), database (PostgreSQL/SQLite/etc.), and ORM/query layer. Add one clarification question if the tech stack is not explicitly stated in the objective.Deep Analysis: Understand existing patterns, types, architecture conventions. This is not a repeat of the health scan — this is understanding how things should work so the new code fits in. For each coupling issue found in Step 1, note it in the affected user story's Technical Specs as:
⚠️ CAUTION: Touches [shared utility/module X] — changes must preserve [behavior Y].Clarification Phase (Karpathy: Think Before Designing): Before writing a single line of the PRD, surface assumptions and tradeoffs explicitly. Don't pick silently:
- If multiple valid architectures exist, present the top 2 and ask which to use
- If scope is ambiguous, state what you'd include/exclude and ask for confirmation
- If a simpler implementation exists that covers 80% of the ask, say so
- Architecture decision log: For every design tradeoff decided in this phase, add one line to
docs/ALWAYS-ON-MEMORY.mdin format:ADR: [decision] — [reason] — [date]Ask the user the following — but only for what cannot be inferred from the codebase or the objective provided: - What is the primary user goal / definition of "done" for this feature?
- Are there external integrations, APIs, or third-party services involved? If yes, specify: the SDK or HTTP client to use, timeout config, error handling strategy, and whether to mock in tests.
- Are there scope boundaries? (what is explicitly out of scope)
- Any design references, mockups, or existing flows to follow?
- Any hard constraints? (deadline, must-not-break, performance budget — for UI features: target Lighthouse score, CLS/LCP/INP thresholds, or bundle size limit)
- If it's a mobile feature: iOS only, Android only, or both? Does this touch navigation (Expo Router screens/layouts), native modules (requires dev-client rebuild), or push notifications (FCM/APNs setup needed)?
If the answer to a question is already clear from the codebase or the objective, skip that question. Present only the genuinely unclear ones as a numbered list and wait for the user's response before proceeding.
Rigor Tier — recommend one, state the one-line reason, let the user override (same pattern as a stack pick — recommend, don't silently choose, don't dump a bare menu):

Recommend a lighter tier only when the signals clearly point that way (explicit "prototype"/"just try it"/"internal tool" language, a scratch repo, no existing users). Record the pick in the PRD header's Rigor Tier field. A single story can still be escalated past the PRD's tier if it individually meets a GA trigger (e.g. one story in an otherwise-Beta PRD touches payments) — note that inline on the story instead of raising the whole PRD's tier.
Greenfield stack confirmation — If the project has no existing codebase, ask ALL of the following before writing any story (these cannot be inferred from an empty repo):

A greenfield PRD built on an unconfirmed stack will require a full rewrite.
Design for Parallelism: Group tasks into Priority levels. Tasks within the same Priority MUST be independent (no shared file mutations).
- Dependency analysis: Before assigning Priority groups, map every cross-story dependency — including implicit ones (e.g., story B calls a function created by story A, or both stories write to the same DB table). If an inter-story dependency is discovered late, move the dependent story to a later Priority group. Output the dependency map as a table in the PRD header:

- Dependency analysis: Before assigning Priority groups, map every cross-story dependency — including implicit ones (e.g., story B calls a function created by story A, or both stories write to the same DB table). If an inter-story dependency is discovered late, move the dependent story to a later Priority group. Output the dependency map as a table in the PRD header:
- Shared utilities: If multiple stories need a new shared helper (function, hook, service class), create a dedicated P1 story for that utility. All stories that consume it go in P2+. The utility story's file is owner-exclusive.
- File conflict resolution: If two stories both need to modify the same existing file (e.g., both need to update
lib/utils.ts), resolve with ONE of these strategies: (a) Split: extract the shared change into a new P1 shared utility story and have both stories import it; (b) Serialize: put one story in P1 and the other in P2 with an explicitdepends_on:link; (c) Merge: if the changes are trivially non-conflicting (different functions in same file), merge both into one story and note the dual responsibility. Never leave file conflicts unresolved — two stories owning the same file is a parallelism bug.- As design decisions are made, update

Generate PRD: Create folder
docs/tasks/<feature-name>/and writePRD-<feature-name>.mdinside it using the template inreferences/PRD_TEMPLATE.md. Also createprogress.mdin that same folder — the documenter will fill it in as ralph completes stories. Pre-Publish Verification Pass — Before sharing the PRD with the user, run these checks:
a. Grep every "uses existing X" claim: Every phrase like "uses the existing service", "calls the current X function", "extends component Y" must be backed by a file path or a grep-confirmable symbol. If unverifiable, mark it inline as

## Breaking Changessection to the PRD header listing each changed field and who consumes it (web clients, mobile, third-party integrations). - Backward compatibility rule: If the change is breaking and there are existing consumers, require an incremental migration: (a) add the new field while keeping the old one, (b) update all consumers in a separate story, (c) remove the old field in a follow-on PRD. Document this explicitly.
- If no consumers are identified (new endpoint or internal-only), write
Breaking change: none. - New endpoint versioning: All new public API endpoints introduced in this PRD MUST declare their version strategy: (a) URL prefix (
/api/v1/,/api/v2/) for REST, (b) query param (?version=2) for minor variants, or (c) header (Accept-Version: 2) for negotiated versioning. If the project has no existing versioning strategy, default to URL prefix and add⚠️ ADR: URL versioning adopted — [date]to the PRD header. Unversioned public APIs cannot be changed without breaking consumers.
b2. Flag composition audit: If the PRD introduces or interacts with any feature flags, add a

Recommend a feature flag wrap for any new feature that: (a) modifies existing shared UI visible to all users, (b) changes an API response shape, or (c) is flagged as risky/experimental in the clarification phase. When wrapping is recommended, add a P1US000: Feature Flag Setupstory.c. Call graph for client→server wiring: If any story wires a client component to server-only code (server actions, API routes, utilities), add a

d. Confirmed in DB for enum maps: If the PRD introduces any mapping keyed on a DB column value (industry, status, tier, objective, etc.), include the confirmed current values:

createTestUser({ role: 'admin' })) in the Testing Strategy section — never hardcode raw object literals inline in tests. The fixture/factory approach ensures tests remain readable and maintainable as schema evolves. If the EPIC's Mission paragraph doesn't use these words, the PRD must not either. Re-read the EPIC's Mission paragraph verbatim before this check.f. Verify minimum 3 ACs per story: Count the AC bullet items for every story. Ensure each story has ≥3 ACs covering (1) happy path, (2) error path, and (3) edge case. Any story with fewer than 3 ACs must be expanded before publishing.
g. Verify story ID uniqueness: Scan all story headers and confirm no duplicate US IDs exist. Each story ID must be unique across the entire PRD. If duplicates are found, renumber before publishing.
h. Verify total story count ≤ 8: Count all user stories in the PRD. If the total exceeds 8, split into multiple PRDs before publishing — one for the current scope and a follow-on PRD for remaining stories. Document the split decision with a note at the top of each PRD referencing the other.
i. Verify rollback commands exist for every Priority group: For each Priority group defined in the PRD (P1, P2, P3, …), confirm there is a corresponding
git revert HEAD~N..HEAD --no-editcommand in the PRD header. If any Priority group is missing its rollback command, add it before publishing.j. CHANGELOG entry: If the feature is user-facing (changes visible UI, adds/modifies API endpoints, or changes behavior observable by end users), add a

CHANGELOG: none (internal only).k. Ban effort estimates: Scan all stories for the words "hours", "days", "estimated", "effort: ", "~Xh", "2h", "ETA". These are prohibited. Replace with parallelism fields (
parallelizable_with,depends_on,critical_path). Time estimates are always wrong, mislead planning, and anchor ralph to artificial deadlines.
Critical Rules
Junior-Proofing
- Never "Implement X" → "Implement X using library Y with these parameters..."
- Never "Update Schema" → "Add field
isActive(Boolean, default true) toUsermodel" - Every user story MUST have Technical Specs with exact code patterns, types, or pseudo-code
- Include
importpaths when referencing existing utilities - Story atomicity: Every user story must be self-contained and completable independently. A story must never require partial completion of another story before it can start. If story B needs a function from story A, they are not in the same Priority group — B goes in P2 after A. Partial completion of a story counts as failure; a story is either 100% done or rolled back.
- No dead code: Never build beyond what the ACs require. If an AC doesn't require it, don't build it. Dead code = scope creep = failed quality gates.
- No TBD or placeholder values: Technical Specs must never contain "TBD", "TODO", "placeholder", or "fill in later". If a value is unknown, surface it as a clarification question BEFORE writing the story, not inside it.
- No
anytypes: Technical Specs must never use TypeScriptany— specify exact types, interfaces, orunknownwith a type guard. If the type is truly unknown, write out the narrowing logic explicitly. - TypeScript strict mode: For TypeScript projects, all Technical Specs assume
"strict": truein tsconfig.json. If the project does not currently have strict mode enabled, add aUS000: Enable TypeScript Strict Modestory as a P1 prerequisite to surface all type errors before new code is added. Never write specs that only type-check withstrict: false.
User Story Template
Every user story in the PRD MUST follow this exact structure. Story IDs use the format US001–US008: the prefix US followed by a three-digit zero-padded sequential number (US001, US002, … US008). Never use US-001, us1, S1, or other formats. The short title (after the ## heading) MUST be written in gerund form (verb-ing phrase), e.g., "Creating user auth module", "Adding payment webhook handler". Never use noun phrases like "User Auth Module" or imperative verbs like "Create user auth".

- Exact function signatures, types, import paths
- Pseudo-code or real code snippets
- Library versions and configuration options
- If new environment variables are required: list each as `ENV_VAR_NAME=<description> (required|optional, type)`. Additionally, include an **env var validation** block in Technical Specs listing all required env vars for this story. Ralph must verify these exist before running the story's code (e.g., in a startup check or Zod `process.env` schema). Missing required env vars at deploy time cause silent failures — they must be caught at startup, not at first use.
- **🔑 REQUIRES SECRET**: Never hardcode API keys, tokens, passwords, or credentials in Technical Specs. If a story requires a new secret, mark it `🔑 REQUIRES SECRET: <VAR_NAME>` and add it to `docs/USER-TASKS.md` so the developer knows to provision it. Hardcoded secrets in a PRD are a security violation and the story will be rejected.
- **Dev environment prerequisites**: If this story requires new tools, services, or setup steps not already in the project's README (e.g., a new Docker container, a new DB, a third-party account), add a `## Dev Setup` block to Technical Specs listing exactly what the developer must run or configure before starting. Example: `docker-compose up postgres redis`, `cp .env.example .env && fill STRIPE_KEY`. Add each setup step to `docs/USER-TASKS.md`. Stories that silently assume the environment is configured cause onboarding failures.
- **Observability**: For stories that add new API endpoints, background jobs, or critical business events, note the observability requirement: log level (info/warn/error), metric to track (e.g., `feature.name.count`), and whether a Datadog/Sentry alert should be configured. If no observability is needed, write `Observability: none`.
- **Rate limiting**: For stories that add or expose public API endpoints, specify whether rate limiting is required. If yes, define: requests per window (e.g., 100 req/min per IP), enforcement layer (e.g., `express-rate-limit`, Cloudflare, Next.js middleware), and the 429 response body. If no rate limiting needed, write `Rate limiting: none`.
- **Pagination**: For stories that add list/collection endpoints (GET /resources), ALWAYS specify pagination strategy: cursor-based (preferred for large datasets) or offset-based, with `limit` default and max (e.g., default 20, max 100). An unpaginated endpoint that returns all rows is a production incident waiting to happen.
- **Error response format**: For stories adding API endpoints, specify the error response shape and use it consistently across all endpoints in this PRD. Recommended: `{ error: { code: string, message: string, details?: unknown } }`. Never mix `{error: string}` and `{message: string}` shapes in the same PRD.
- **Input validation**: For stories adding forms or API mutation endpoints, specify the validation library (e.g., Zod, Yup, valibot), schema location (e.g., `lib/schemas/feature.ts`), and which fields are required vs optional with their constraints (min/max length, pattern, enum values). Never leave validation as "validate the input" without specifics.
- **Security review flag**: If this story: (a) adds/modifies auth middleware, (b) changes access-control rules, (c) handles PII or payment data, (d) adds file upload/download, (e) adds a new public API endpoint without authentication, or (f) receives inbound webhooks from external services — add `⚠️ SECURITY REVIEW REQUIRED` as the first line of Technical Specs and set `security-review: true` in the **Files** block. For webhook receivers (f): Technical Specs MUST also specify the HMAC webhook signature verification method (e.g., verify `x-signature` header with `crypto.timingSafeEqual`, or use `stripe.webhooks.constructEvent`), the secret env var (`🔑 REQUIRES SECRET: WEBHOOK_SECRET`), and HTTP 400 response on signature mismatch — an unverified webhook is an unauthenticated attack surface. Stories without this flag are assumed safe to ship without security review.
### Testing Strategy
- How ralph should test this story: unit tests, integration tests, or e2e (playwright)
- List any mocks/stubs required (e.g., mock external API, stub DB call)
- Specify the test file path(s) that ralph must create BEFORE writing implementation code
- **Coverage gate**: Stories that add business logic (services, utilities, server actions) must achieve **≥80% line coverage** on their added files. Verify with `pnpm coverage --reporter=text`. If the project has no coverage tooling configured, note this as a USER-TASK. Stories that only add UI without logic (pure presentational components) may write snapshot tests instead of coverage gates.
### Acceptance Criteria
Each AC item MUST be written as a testable assertion and **numbered** (AC1, AC2, AC3…) so spec-writer can reference them by ID in the generated test spec:
- [ ] **AC1**: Navigate to <URL> → expect <specific visible element or text>
- [ ] **AC2**: Submit form with <data> → expect <response or state change>
- [ ] **AC3**: With <invalid/edge case input> → expect <graceful error, not crash> ← at least one edge case REQUIRED
- [ ] **AC4**: <business rule> → expect <verifiable outcome>
**Minimum 3 ACs required**: (1) happy path, (2) error/failure path, (3) edge case or boundary condition. Stories with fewer than 3 ACs will be rejected.
**Empty state required for collection stories**: If the story renders a list, table, or collection of data, one AC MUST cover the empty state: `With 0 items in the collection → expect <empty state message or illustration> visible, not a blank page or broken layout`.
**Loading state AC required for async UI stories**: If the story fetches data asynchronously or performs async mutations (API calls, form submissions, file uploads), at least one AC MUST cover the loading state. Example: `While data is loading → expect skeleton component or spinner visible; data table must not flash empty before populating`. Specify the exact skeleton component path (e.g., `<TableSkeleton />`) in the Technical Specs.
**Error boundary for React/Next.js UI stories**: If the story adds a React component that renders async data or performs side effects, the Technical Specs MUST specify the error boundary strategy: (a) which error boundary wraps this component (existing `<ErrorBoundary>` from layout or a new one), (b) what the fallback UI shows (error message + retry button vs redirect), and (c) whether errors are reported to Sentry/Datadog. A component without an error boundary spec will white-screen on uncaught exceptions.
**Accessibility (a11y) required for UI stories**: If the story adds or modifies user-facing UI (forms, buttons, modals, navigation, lists), at least one AC MUST verify accessibility: keyboard navigability, ARIA labels, or WCAG 2.1 AA compliance. Example: `Tab to the Submit button → expect focus ring visible and `aria-label` present`. Use `axe-core` or `playwright-axe` for automated a11y assertions. Stories that add interactive elements without an a11y AC will be rejected.
AC items written as vague conditions will be REJECTED. Prohibited phrases: "works correctly", "displays properly", "handles correctly", "validates properly", "functions as expected", "is shown", "appears". Each AC must name a specific URL, element, value, or status code.
Good AC example: `- [ ] POST /api/users with valid body → expect HTTP 201 and response body includes { id, email }` — e.g., Navigate to /dashboard → expect `<h1>Dashboard</h1>` visible in DOM.
Every AC must be completable with: playwright-cli + visual check OR pnpm test assertion.
Parallelism Design
- Tasks in the same Priority group MUST NOT modify the same files
- Each task declares its Files (owned files that only this task touches)
- If two tasks need the same file, they go in different Priority groups
- The orchestrator will launch all tasks in a Priority group simultaneously
- Failure handling: If any story in a Priority group fails quality gates, the entire group should be rolled back before proceeding to the next Priority group. Document the rollback command in the PRD header using this format:
Example: if Priority 2 had 3 stories, the rollback command is# Rollback Priority N group: git revert HEAD~<N-stories-in-group>..HEAD --no-editgit revert HEAD~3..HEAD --no-edit. The PRD header must include one rollback command per priority group. - Story failure criteria: A story fails if: (a)
pnpm typecheckorpnpm lintexits non-zero after its changes, (b) any AC-derived test fails, OR (c) the story created/modified files not declared in its Files field. - Fail fast on Priority group failure: If ANY story in a Priority group fails its quality gates, STOP — do not start the next Priority group. Roll back the entire failed group using the PRD header rollback command, fix the failing story, then re-run the group from scratch. Partial Priority group progress must never be carried forward.
Quality Gates
- Define quality gate commands in the PRD header (e.g.,
pnpm typecheck,pnpm lint) - Every user story's acceptance criteria must be verifiable by running these commands
Scope Control
- Max 8 user stories per PRD. If more, split into multiple PRDs.
- Each user story should be completable in ~30 min by a dev agent
- Story too complex? Split if: (a) it touches >3 files, OR (b) it mixes concern layers (e.g., schema + API + UI in one story → separate into 3 stories), OR (c) its AC list exceeds 5 items
- Schema changes: If any story requires a DB schema change (new table, column, or index), create a dedicated P1 story
US000: DB Migrationthat owns only the migration file. No other story may touch the schema file directly. Migration file naming must follow the project convention — if using Drizzle:drizzle/migrations/<timestamp>_<feature-name>.sql; if using Prisma:prisma/migrations/<timestamp>_<feature-name>/migration.sql. The timestamp must be the current UTC datetime inYYYYMMDDHHMMSSformat to ensure sequential ordering.- Destructive migration warning: Any migration that drops a column, renames a column, or drops a table is a destructive migration. - DB transaction requirement: If any story writes to 2+ tables in a single operation (e.g., create user AND create profile), the Technical Specs MUST require a database transaction to prevent partial writes. Specify: ORM transaction method (e.g., Drizzle
db.transaction(), Prisma$transaction([])), which operations are grouped, and the rollback behavior on error. - Background jobs & async patterns: If a story adds a cron job, queue consumer, or background worker, its Technical Specs MUST specify: (a) trigger mechanism (cron schedule in UTC, queue topic/event name, or webhook payload), (b) idempotency strategy (how duplicate triggers are detected and ignored), (c) failure behavior (retry policy: max attempts, backoff strategy, dead-letter queue), (d) observability (log on start/success/failure, metric name), and (e) cold start / initialization time: if the job requires loading large models, establishing DB connection pools, or warming up caches on first invocation, document this startup cost and whether the job platform (Lambda, Cloud Run, etc.) supports keep-warm strategies. A background job story without these five fields is incomplete and will be rejected.

If blocked at any point (missing critical info, unresolvable ambiguity), emit instead:

The `<reason>` must specify: (1) what information is missing, (2) who can provide it (user, codebase, external service), and (3) what the architect will do once it is provided. Example: `ARCHITECT_BLOCKED: Cannot determine auth strategy — user must specify whether to use Better Auth or Lucia. Once confirmed, will design US002 auth middleware story.`
**Never emit ARCHITECT_BLOCKED for things the architect can decide independently.** Only block when: (a) the decision would reverse major architectural choices if guessed wrong, (b) the missing info cannot be inferred from the codebase or objective, or (c) the feature has legal/compliance implications requiring explicit sign-off. When in doubt, make a reasonable default, document it as an ADR, and proceed.
