PROBLEM STATEMENT FORMULA:

EXAMPLE:

SIPOC DIAGRAM:
Suppliers -> Inputs -> Process -> Outputs -> Customers
Map the high-level process to align the team on scope.

Measure

MEASUREMENT SYSTEM ANALYSIS:
Before collecting data, verify your measurement system is reliable.
- Can different people measure the same thing consistently? (Reproducibility)
- Can the same person measure consistently over time? (Repeatability)
- If measurement system variation is high, fix it before collecting data.

Analyze

ROOT CAUSE ANALYSIS TOOLS:

PARETO CHART:
- Rank defect categories by frequency
- 80/20 rule: 80% of defects from 20% of causes
- Focus improvement on the vital few, not the trivial many

FISHBONE (ISHIKAWA) DIAGRAM:
Categories: Man, Machine, Method, Material, Measurement, Environment
- Brainstorm potential causes in each category
- Identify the most likely root causes for testing
- Validate with data, not opinion

5 WHYS:
Problem: Orders shipped to wrong address
Why 1: Address label was incorrect -> Why?
Why 2: CSR entered the wrong address -> Why?
Why 3: Customer gave address verbally -> Why?
Why 4: Online system was down, used phone -> Why?
Why 5: System has no redundancy for address validation

HYPOTHESIS TESTING:
- Use data to test whether a suspected cause is actually significant
- Chi-square test: for categorical data (defect types by shift)
- t-test: comparing two group means (before/after, A vs B)
- Regression: relationship between variables (temp vs defect rate)
- Do not guess at root causes - test them statistically

Improve

SOLUTION DEVELOPMENT:
1. Generate potential solutions for validated root causes
2. Evaluate solutions using a criteria matrix:
   - Effectiveness (will it fix the root cause?)
   - Feasibility (can we actually implement it?)
   - Cost (is it affordable?)
   - Speed (how fast can we implement?)
   - Risk (what could go wrong?)

3. Pilot the top solution(s) before full rollout
   - Run a controlled pilot (test group vs control group)
   - Measure the same metrics as baseline
   - Use statistical tests to confirm improvement is real

IMPLEMENTATION PLAN:
- What changes are being made (specific process changes)
- Who is responsible for each change
- Timeline for implementation
- Training requirements
- Communication plan
- Risk mitigation for implementation

Control

CONTROL PLAN ELEMENTS:
- Updated process documentation (SOPs)
- Control charts for ongoing monitoring
- Response plan: what to do when process goes out of control
- Training for process operators
- Audit schedule
- Ownership and accountability

STATISTICAL PROCESS CONTROL (SPC):
- Control charts track process performance over time
- Upper Control Limit (UCL) and Lower Control Limit (LCL) set at 3 sigma
- Points within limits = process is in control (common cause variation)
- Points outside limits or patterns = out of control (special cause)
- React to special causes, do not over-adjust for common causes

CONTROL CHART RULES (Western Electric Rules):
Out of control signals:
1. One point beyond 3 sigma (UCL or LCL)
2. Eight consecutive points on one side of center
3. Six consecutive points steadily increasing or decreasing
4. Two of three consecutive points beyond 2 sigma (same side)

SUSTAINING IMPROVEMENTS:
- Monitor weekly/monthly (do not stop measuring)
- Conduct quarterly audits of the control plan
- Retrain staff when turnover occurs
- Update control plan when process changes
- Celebrate sustained performance (reinforce the new normal)

Project Selection Criteria

GOOD SIX SIGMA PROJECTS:
- Clear, measurable problem with available data
- Process is stable (not in chaos - fix chaos first)
- Problem is chronic, not a one-time event
- Business impact justifies the investment
- Root cause is not already known (if you know it, just fix it)
- Scope can be completed in 3-6 months

POOR SIX SIGMA PROJECTS:
- "Improve everything" (too broad)
- Solution already determined (not data-driven)
- No data available and none can be collected
- Political problem disguised as a process problem
- Too small to matter or too large to scope

PROCESS CAPABILITY:
Cp = (USL - LSL) / (6 x sigma)
  Measures: Can the process meet specifications (potential)?
  Cp >= 1.33: capable. Cp < 1.0: not capable.

Cpk = min[(USL - mean) / (3 x sigma), (mean - LSL) / (3 x sigma)]
  Measures: Is the process actually meeting specs (performance)?
  Cpk >= 1.33: good. Cpk < 1.0: producing defects.

Cp = Cpk when the process is perfectly centered.
If Cp is high but Cpk is low: process is capable but not centered.
Solution: shift the process mean (easier than reducing variation).
