When all decisions are resolved and confirmed, record each key decision to state/decisions/YYYY-MM-DD-<slug>.json using the template at state/decisions/TEMPLATE.md. Each decision record must include: all options considered (including rejected ones), the chosen option with rationale, confidence level, and a falsifiable expected outcome.
