Clarification Rule

If the user has not adequately specified the prognostic biomarker question, this skill must clarify the minimum items needed before locking the design:

• disease / condition / clinical context
• intended target population and disease stage
• biomarker modality or candidate feature space
• whether the marker is measured at baseline or a landmark time
• intended prognostic endpoint
• likely follow-up horizon
• available data type and sample source
• whether external validation data may exist

If critical inputs are missing, ask 2–6 concise, high-yield follow-up questions.

Do not ask a long questionnaire if a narrower set of questions would establish:

• whether the biomarker use is truly prognostic
• what the primary endpoint and time scale are
• whether the design is single-marker, panel, or multivariable score based
• what validation architecture is realistic

If the user wants a one-shot protocol framework, proceed with explicit assumptions and label assumption-dependent elements clearly.

Supported Prognostic Biomarker Study Families

The skill must first identify the dominant study family. Typical families include:

If the user’s idea could fit more than one family, explicitly identify the lead family and the main alternative.

Prognostic Design Selection Logic

Choose the design form based on the prognostic use case, endpoint timing, cohort reality, and marker dimensionality, not by habit.

Typical mappings:

• Single biomarker prognostic study → one pre-specified marker with a clear biological or clinical rationale and a limited set of adjustment variables
• Multi-marker panel study → a modest number of candidate markers to be combined into a prognostic score or grouped signature
• High-dimensional omics score study → a broad feature space requiring stronger preselection, shrinkage, internal validation, and disciplined external validation
• Integrated clinicomolecular model study → biomarker information combined with established clinical covariates
• Validation-first study → focus on reproducing a previously proposed biomarker or score in an independent cohort before redesigning the marker set

Prefer the simplest protocol family that can answer the user’s real objective.

Execution

Step 1 — Clarify the true prognostic use case

• the disease context
• the intended prognostic use
• whether the marker is baseline or landmark-based
• whether the endpoint is survival, recurrence, progression, mortality, or another clinical event
• what non-prognostic interpretations must be excluded

Step 2 — Define the cohort backbone and data reality

State:

• source population
• cohort entry logic
• sample / biospecimen / assay timing
• retrospective versus prospective structure
• discovery, validation, and possible external cohorts
• what data elements are truly available versus only assumed

Step 3 — Define endpoint family and time scale

State:

• primary prognostic endpoint
• key secondary endpoints
• time origin
• follow-up horizon
• censoring logic
• competing-event structure where relevant
• whether the endpoint should be modeled as time-to-event, fixed-horizon binary risk, or longitudinal worsening/progression

Step 4 — Select the lead study family

Map the study to one dominant prognostic biomarker family and one main alternative.

Explain why the recommended family best matches:

• marker dimensionality
• event structure
• likely sample size / event count pressure
• desired interpretability
• validation realism

• when model updating or recalibration may be needed

Step 8 — Audit overfitting, leakage, and instability risk

Review threats such as:

• data leakage from feature selection across the full dataset
• outcome-informed threshold picking
• low events-per-parameter pressure
• unstable cutpoints
• batch or platform effects
• post-baseline contamination
• optimism from reusing the same cohort for discovery and validation
• overinterpretation of discrimination without calibration

Step 9 — Check translation readiness and next-step realism

Use references/clinical-translation-readiness-rules.md.

State clearly:

• whether the biomarker is only discovery-stage, model-development stage, or validation-ready
• whether the assay platform and turnaround are realistic
• whether incremental clinical value is demonstrated or only hypothesized
• whether external implementation should be deferred pending stronger validation

Step 10 — Recommend the lead protocol version

Choose the best protocol framing for now.

State:

• the recommended design version
• why it should lead
• what has been intentionally deferred
• what upgrades would strengthen the study later
• whether the protocol is firm or provisional

Mandatory Output Structure

Use the following sectioned structure every time.

A. Study Intent Summary

Provide a concise restatement of the user’s prognostic biomarker question, intended use, biomarker modality, and target outcome.

B. Why Prognostic Biomarker Design Fits

State whether the request is truly prognostic, what competing biomarker-use families were considered but not selected, and what interpretation level the design can support.

C. Recommended Study Family

State the recommended prognostic biomarker study family, the main alternative, and the design trade-off.

D. Cohort Backbone and Follow-up Architecture

Define source population, eligibility backbone, measurement timing, cohort entry, time origin, follow-up structure, and censoring concept.

E. Endpoint and Time-Scale Framework

Define the primary endpoint, key secondary endpoints, endpoint timing, operational definitions, and whether the primary analysis should be time-to-event, fixed-horizon, or longitudinal.

F. Candidate Marker and Variable Framework

Organize the biomarker and covariate system into required domains. This section should separate core pre-specified markers and covariates, recommended enrichment variables, and optional exploratory variables.

G. Model Development and Risk Stratification Plan

State the main modeling target, model family, covariate strategy, threshold / grouping logic, and key performance priorities.

State which assumptions depend on assay availability, event accrual, sample size, platform harmonization, follow-up quality, or access to independent cohorts.

K. Recommended Protocol Version

Give the lead protocol recommendation and explain why it is the best version to execute now.

L. Critical Assumptions and Next Clarifications

List the assumptions that still require confirmation and the minimum follow-up questions or decisions needed before the protocol becomes execution-ready.

Formatting Expectations

Follow these formatting rules every time:

• Keep the response sectioned exactly as A–L.
• Use concise paragraphs for interpretation sections.
• Use tables where structure comparison improves clarity.
• The following sections should usually use tables unless the input is extremely simple:

Hard Rules

Study-Design Integrity Rules

• Do not call the study prognostic unless the intended use is outcome-risk estimation independent of treatment allocation.
• Do not blur prognostic biomarker design with diagnostic or predictive biomarker design.
• Do not use post-baseline or post-treatment variables as baseline prognostic predictors without explicitly labeling the bias risk.
• Do not recommend multiple competing primary endpoints without naming one true primary endpoint.
• Do not give an endpoint label without an operational definition and time scale.
• Do not treat univariable association as sufficient evidence for a clinically useful prognostic biomarker.
• Do not recommend a complex high-dimensional model unless the event structure and validation plan can plausibly support it.
• Do not imply that risk groups are robust if cutoffs are data-driven and not yet validated.
• Do not present discrimination alone as adequate prognostic validation; calibration and generalizability must also be considered.

Feasibility and Data Rules

• Do not invent cohort size, event count, follow-up duration, assay success rate, external validation access, or biomarker measurement completeness.
• Do not assume omics, pathology, imaging, ctDNA, proteomics, or longitudinal biospecimen data are available unless the user said so or the output explicitly labels them as assumption-dependent.
• Do not assume standardized assay platforms, batch-correction success, or cross-center harmonization.
• Do not silently rely on unavailable clinical covariates for the core model backbone.
• Do not assume enough events exist to support feature-rich model development.

Literature and Evidence Integrity Rules

• Never fabricate references, PMIDs, DOIs, trial IDs, cohort names, registry names, assay validation status, event rates, guideline positions, or published precedent.
• Never imply that a biomarker, score, threshold, or signature is clinically established unless that is actually verified.
• Never state that external validation has been done unless confirmed.
• If literature support is not verified, say so explicitly.
• If expected event rate, follow-up completeness, or assay reproducibility is unknown, label it as unknown rather than guessed.

Output Discipline Rules

• Always provide one lead protocol version.
• Always separate necessary, recommended, and optional markers or design components where applicable.
• Always identify the strongest leakage or overfitting risk.
• Always surface the assumptions most likely to fail in real data.
• Always keep the protocol compatible with the user’s stated question rather than inflating it into a more ambitious but less executable biomarker program.

Interactive Refinement Rule

If the user asks to improve or revise the protocol, preserve the same A–L output structure unless they explicitly request a different format.

When refining:

• keep the original core question stable unless the user changes it
• state what changed in the revised design
• explain why the change improves interpretability, robustness, feasibility, or generalizability
• do not add complexity unless it solves a concrete design problem

What This Skill Should Not Do

This skill should not:

• act as a patient-care prognostic counseling tool
• write grant prose, manuscript text, or regulatory submissions unless explicitly asked in a later workflow
• generate sample-size calculations from fabricated event-rate assumptions
• produce literature citations unless they are verified
• redesign a diagnostic or predictive biomarker task while still calling it prognostic
• collapse the entire study into a purely mechanistic omics workflow without a prognostic endpoint backbone
• treat every available feature as modeling-eligible

Quality Standard

A high-quality output from this skill should:

• make clear why the chosen prognostic study family fits the question
• define a defensible time origin, endpoint framework, and follow-up structure
• provide a usable candidate-marker and covariate framework
• present one coherent model-development and risk-stratification line
• define an honest validation architecture
• expose the main threats to leakage, optimism, and transportability
• remain useful even if the user has not yet finalized all operational details
• never overstate certainty, clinical utility, data availability, or validation maturity
