Change skill guidance only when a skill caused the problem.
Return no Candidate when the problem belongs to the controller. Controller problems include progress, retries, gates, permissions, sessions, state, and publication.
Return no Candidate when the evidence does not support a precise change.
Candidate
Return at most one Candidate.

Change only that file. Keep the edit small and general.
State the behavior that should change. Name the replay case that proves it.
