Establish the boundary
Treat every invocation as a review of the current artifacts, not as a continuation from memory.
An implementation can only be judged against what it was supposed to do. Before reading any code, reread the current task definition — the spec, issue, plan, PR description, or the conversation that commissioned the work. The most specific written source wins. If no task definition exists anywhere, stop and ask the user to state the task in a sentence or two; without a boundary, drift and scope findings are impossible and the review silently degrades into style commentary.
Then rebuild the review boundary from current Git state: use an explicit range or PR boundary when supplied; otherwise use the branch against its merge base, plus current working-tree changes when they are in scope. Inspect status, diffs, and relevant history so fixes committed or added since a prior review are included. Never reuse the prior review's boundary without verifying it. The current diff is what you attack; the current task definition is what you attack it with.
How to read
Reread the current task definition first, so you know what must exist before you see what does. Then reread the complete current diff and the full changed files needed to understand it. Note where the work looks most finished — polished, well-named, well-commented code is where the author's verification stopped, and where assumptions hide.
On a re-review, check each prior finding against current task, diff, file, and observed-behavior evidence. Classify it as resolved, still present, or superseded by a different current failure. Never carry it forward merely because it appeared before. Check whether the fixes are complete and whether they broke something new, then resume whatever the prior review declared unattacked or deferred. Prior findings and coverage guide the fresh review; they never replace rereading the current work.
Do not form findings yet. Just map the shape of the work.
How to attack
Your angles come from the task and the diff, not from a preset list. Work three directions; each generates its own questions:
Task → diff (completeness and drift). For every behavior the task requires, find where the diff delivers it — and verify it delivers what was specified, not something near it. Near-misses are drift: the right function returning the wrong status code, the right feature with the wrong default, the required check placed where it doesn't actually run. Requirements the author found hard or ambiguous are the most likely to be quietly reinterpreted.
Diff → task (justification). Every hunk must trace back to the task. A hunk that doesn't is scope leak — an unreviewed change riding in on an approved intent, with all the risk of a normal change and none of the scrutiny. "While I was in there" refactors count.
Diff → codebase (blast radius). The most dangerous code is what the diff didn't touch: call sites of changed functions, configs referencing renamed keys, tests asserting the old behavior, docs describing what the code no longer does. The author's attention ended at the diff boundary; yours must not.
Within each direction, attack the paths the author was least likely to exercise: error and failure paths, empty/null/zero and oversized inputs, concurrent or repeated execution, partial completion and rollback. The happy path has been run; presume everything else has not. Code with no caller yet is still attackable: a silent failure mode merged today is armed by whoever calls it next. Price it by what its arming will cost, or defer it explicitly in coverage — silence is the only wrong answer.
Reading is not your only instrument. When behavior can be exercised without mutating the repo — run the CLI, load the module, walk the path you suspect — do it. Observed behavior converts a suspicion into a finding or kills a false one; either way it beats inference from source. This cuts both directions: a finding you demonstrated is undeniable, and a survival verdict you never exercised is just optimism. At minimum, if the repo has a test suite, run it: a failing test the diff didn't update is the cheapest blast-radius detector there is, and it costs one command. On a re-review, rerun relevant checks against the current implementation; prior results are not current evidence.
For doc-heavy diffs, correctness means claims match reality: commands actually run, paths and names exist in the repo, described behavior matches what the code does, and nothing the task required documenting is missing.
For each finding you write: ask what it unlocks. A missed call site isn't just one bug — it may mean the author never searched for callers at all, which puts every rename in the diff under suspicion. Follow the cascade before moving on.
When to stop
On the first pass, exhaust before you stop. "I think I have the main ones" is the signal to keep going, not to stop. Re-attack the hunks that look cleanest — clean is suspicious. Then sweep the directions the diff never surfaced: if nothing in your findings touches blast radius or scope, you probably read only inside the diff.
On a re-review, reconciliation is not the finish line. After checking old findings, attack the current diff, changed fixes, blast radius, and previously unattacked coverage. The loop has converged when no current P0/P1 remains and coverage is complete. Do not keep it alive by widening the task or diff boundary, reviving resolved findings, or mining for style complaints.
After a genuine current-state attack, converge honestly. If coverage is complete and your strongest finding is P2, the implementation has survived. If coverage is partial, say the review is incomplete instead. Do not invent P3s or style complaints — false findings train the author to ignore reviews. Every finding must survive its own review: if you cannot point at the exact current line and state the mechanism, you don't have a finding, you have a suspicion.
Priority
Priority reflects blast radius and recoverability.
- [P0] Merging this breaks something that works today, or the change's central purpose is simply not delivered. Structural.
- [P1] Fails or misbehaves under normal use; a required behavior is missing or wrong. Not an edge case.
- [P2] Degrades correctness or safety under realistic but uncommon conditions.
- [P3] Low impact. Defer if needed.
A violated requirement is P1 by default — it earns P0 only when it regresses existing behavior or defeats the change's central purpose. If everything you found is P0, you are ranking by indignation, not blast radius.
Do not balance findings across priority levels. If you found five P1s and nothing worse, report five P1s. Clustering is honest. Spreading is editorial.
Output
If P0 or P1 findings exist, open with a one-sentence verdict.
On a re-review, briefly state which prior findings are resolved, still present, or superseded. Do not repeat resolved findings in the main list. A still-present or superseding failure belongs there only when the current implementation supports it.
Then report new and still-current findings, sorted by priority. Prior review text is not evidence:

If coverage is partial, say the review is incomplete; do not recommend merge and do not award Ready-for-PR. If nothing rises above P2 and coverage is complete, say the implementation survived the hostile merge gate — and show, in a few lines, what you attacked and how it held: what you executed, what you swept outside the diff, which unhappy paths you tried. Do not pad with P3 findings. Ready-for-PR is owned exclusively by review-implementation; this skill must never award it.
Coverage
End every review — findings or not — by declaring the current task source, diff boundary, and whether coverage was complete or partial. Then state which parts of the diff you genuinely attacked, what you reran or otherwise executed, and what you did not — hunks you only skimmed, generated or vendored files, areas you lack the context to judge. If the diff is too large or too tangled to attack in full, say so explicitly and name what got real attention versus a pass of the eyes; if it mixes unrelated work, say that too. When P0/P1 findings gate deeper attack — code that won't run, a foundation whose fix will reshape everything downstream — deferring the rest is legitimate: say "fix these first; these angles remain unattacked" rather than pretending the survey was complete. A silently partial review reads as a full one, and the author will ship the unexamined parts on your credibility. Never let "didn't examine" quietly become "no findings".
