Procedure
Identify the inputs. State each source with its date and author role; if several items appear, ask which one this report covers or propose one report per item. Confirm the set before drafting: a workflow hold that the typed confirmation releases and that authorises nothing else.
Build the evidence table from every input (columns under Output). Copy measurements with units, tolerances and instrument identifiers exactly; never round, average or convert.
Identify the item: every field from input 3 with its source or UNKNOWN. Quantity affected and quantity inspected are separate; a missing one is UNKNOWN, never assumed equal to the other.
Describe the nonconformity in one factual paragraph in the notes' wording: what was observed, where on the item or site, when, by which role, and the detection point. No cause words (because, due to), severity words (critical, major, minor) or judgement words (unacceptable, defective, failed) except inside a labelled quote.
Cite the requirement and state the departure: for each requirement the observation departs from, quote the clause verbatim with document, revision and clause or sheet, beside the observed value and the departure (the plain arithmetic or visual difference, no adjective). Where the document is absent or the clause cannot be found, Requirement UNKNOWN with the document named; never reconstruct a clause from memory.
If the governing document maps departures to categories, reproduce the criteria beside the observed value and write "classification: for the quality lead". Never pick the category.
Propose containment from the reference's catalogue. Each proposal names the role who would perform it, what would show it done, and the procedure clause that calls for it, or "no procedure clause found". Proposals only; the quality lead directs.
Similar-item exposure: other items, batches or locations the sources suggest could share the departure, each as a question with its source. Never state they are affected.
Disposition as a blank decision block: the options the procedure lists, the approval each requires, and "disposition: for the quality lead or review board". Do not recommend.
