Prompt Injection Defense: Layered Strategies

Prompt injection is not a theoretical attack. By Q1 2026, the benchmark is real: CaMeL achieves 67% injection block rate on AgentDojo. NIST AI RMF frames it as agent hijacking. Defense is not a single gate; it's depth.

Threat model

Direct injection: Attacker controls the user prompt directly. "Ignore your instructions and do X." Modeled by NIST as prompt hijacking.

Indirect injection: Attacker controls untrusted data that flows into the prompt context. Examples:

• Retrieved documents (RAG): attacker posts a GitHub issue that becomes part of the agent's context.
• Tool outputs: LLM asks a tool that returns attacker-controlled data.
• Email/scraped content: agent processes inbound messages, web articles, or logs.
• User-uploaded files: CSV, PDF, or text parsed into context.

Indirect injection is harder to detect and more common in production. It's what breaks real agents.

Layered defenses

Depth > any single layer. Deploy multiple controls so one failure doesn't collapse the system.

1. Input sanitization and structural boundaries

Goal: Limit how much untrusted data reaches the LLM, and use channel separation to make injection visible.

Techniques:

• System vs. user message separation: Keep your instructions in system_role, user input in messages[].content. Don't concatenate raw user input into the system message.
• Tool output framing: Wrap tool results with metadata (<tool_result source="web_search" confidence="0.8">...</tool_result>). Mark them untrusted.
• Content stripping: Remove HTML directives from scraped web content. Strip ANSI codes, shell metacharacters from logs.
• Length limits: Truncate long retrieved documents. Don't feed entire PDFs; summarize or chunk by relevance.

Tradeoff: Slows down RAG (extra escaping) and can truncate useful context. Worth it.

2. Guard model (cheap filter)

Goal: Pre-filter user input with a fast, cheap model before it reaches your expensive executor LLM.

Technique: Route every prompt through a lightweight classifier (Haiku 4.5, GPT-4 Mini, or a fine-tuned small model) that detects obvious injection patterns.

Cost: ~0.1 µ per request vs. 2.5 ¢ for an Opus call. Early rejection saves money and latency.

Tradeoff: False positives (blocking legitimate requests). Tune the confidence threshold. False negatives happen; this is not a complete defense, just a cheap filter.

3. Output validation and refusal enforcement

Goal: Ensure the LLM's response is safe before executing tools or returning to the user.

Techniques:

• Schema enforcement: Require structured output (JSON). Parse and validate against a schema.
• Refuse tokens: If the LLM says "I can't do that," respect it. Don't retry or rephrase.
• Known-answer detection: For facts you control (your company name, product behavior), verify the LLM didn't make up false answers that could confuse users.

Tradeoff: Strict schema enforcement can limit agent creativity. Start strict, relax as you understand the failure modes.

4. Tool allowlists and capability scoping

Goal: Constrain what the LLM can do. Even if injection succeeds, the blast radius is limited.

Techniques:

• Function allowlist: Only expose tools the user's role needs. Don't give every agent access to delete_database.
• Per-request tool budget: Allow max 3 tool calls per agent turn, max 5 turns total before human review.
• Tool-level auth: Tool calls must pass per-tool permission checks (e.g., "web_search only for domain whitelist").

Tradeoff: Limits agent autonomy. Accept it. A constrained agent that doesn't get hijacked is better than a free agent that does.

5. Sandboxing untrusted content

Goal: Isolate untrusted inputs so they can't access secrets or critical systems.

Technique: Run tool calls and context retrieval in a sandbox with reduced permissions. Link to packages/core/patterns/agent-sandboxing.md.

• Separate context window: If a retrieved document is hostile, it shouldn't have access to your conversation history or previous queries.
• Read-only filesystem: Tools inside the sandbox can only read, not write to critical paths.
• Network restrictions: Tool outputs from external APIs are fetched in a sandboxed process; attacker can't access internal services.

Tradeoff: Overhead (sandboxes add latency). Use only for untrusted data sources.

6. Audit logs and human-in-the-loop for high-risk actions

Goal: Detect hijacking in progress and enforce human sign-off for dangerous tool calls.

Technique:

• Log every LLM decision: What tool did it call? With what arguments? Did the response look normal?
• Anomaly detection: If the agent suddenly calls delete_user (which it never did before), flag it.
• Require approval for risky tools: modify_permissions, delete_records, deploy_code require explicit human approval before execution.

Tradeoff: Blocks on human response. Acceptable for production systems.

7. Indirect injection: mark and isolate untrusted sources

Goal: Specifically defend against injection via RAG, tool outputs, and external data.

Techniques:

• Source labeling: Every piece of external data carries metadata about origin and trustworthiness.
• Instruction stripping: Retrieve docs as plain text, not markdown. Remove headers like # NOTE: ADMIN MODE ENABLED.
• Context isolation: Each retrieved document gets its own prompt section, clearly marked as external.

Tradeoff: Loses formatting richness. Information still gets through; formatting is a luxury.

Known attacks and detection

What doesn't work

Single-prompt refusal: "Ignore previous instructions" is the attack, not the defense. A single system message prompt that says "don't do X" will fail under social engineering.

One-layer defense: A guard model alone is not enough. A tool allowlist alone is not enough. Use all layers; assume one will fail.

Trusting the model to self-restrict: The model will try to comply with the attacker's instructions if they're sufficiently obfuscated. Defense is external: structure, validation, audit, human review.

Sanitizing user prompts only: Indirect injection comes through retrieved docs, tool outputs, emails, and uploaded files. You must sanitize all untrusted data, not just the user's direct input.

Deployment checklist

• Guard model: Classify user input with a cheap classifier. Threshold at 0.7+ confidence.
• Input sanitization: Wrap all retrieved/external content with source labels and length limits.
• Tool allowlist: Define which tools each user role can access. Enforce in code.
• Output validation: Parse structured responses; validate tool calls against whitelist.
• Audit logging: Log every tool call, user, arguments, results. Monitor for anomalies.
• High-risk approval: Require human sign-off for delete, modify_permissions, deploy operations.
• Sandbox untrusted sources: Run web searches, file parsing, API calls in sandboxed processes.
• Monitoring: Track false positives from the guard model. Refine thresholds monthly.
