An empty hygiene column is rarely a marketing problem. It is a list problem — nobody has segmented who actually lapsed and why — and a friction problem, because rebooking requires a phone call during working hours. This builds the list, writes the message that fits each reason for lapsing, and removes the steps between wanting an appointment and having one.

Required Inputs

Ask for these if not provided:

• The list — how many patients are overdue and by how long, and what your practice management system can segment on
• The history — why patients have lapsed, if known: cost, fear, moved, dissatisfaction, or simply drift
• The channels available — SMS, email, phone, post, and whether you have consent for each
• The capacity — how many hygiene slots need filling, and when
• The constraints — regulatory rules on patient contact and marketing in your jurisdiction, and who has time to make calls

Framework: Segment by Reason, Not by Date

1. Split the overdue list by why, not just by how long. A patient 8 months overdue after a painful visit needs a different message from one who simply forgot.
2. Start with the warmest segment. Recently lapsed and previously regular converts far better than three-years-gone; do it first so the effort funds the rest.
3. Write to the reason. Fear gets an acknowledgement and a low-commitment offer. Cost gets transparency about what a check-up actually costs. Drift gets a simple, specific prompt.
4. Make booking one step. A link that shows real slots beats 'call us to arrange'. Every step removed is measurable.
5. Set a stop rule. Three contacts across two channels, then move to a dormant list. Beyond that you are training people to ignore you.
6. Measure returns, not sends. Open rates are vanity; booked and attended appointments are the number.

Operations and communications only. Recall intervals are a clinical decision for the treating clinician based on individual risk, not a scheduling default. Patient-contact consent, marketing rules, and data-protection obligations vary by jurisdiction — check before any campaign.

Quality Checks

• Segments are defined by reason for lapsing, not only by months overdue
• Each segment has its own message rather than one template sent to everyone
• A stop rule exists and is stated
• Booking friction is counted in steps before and after
• Success is measured in attended appointments, not messages sent
• No clinical recall interval is asserted

Anti-Patterns

• One message to the whole overdue list. It reads as bulk, converts poorly, and burns the list.
• Contacting without a stop rule. Persistence past three attempts costs you the patient permanently.
• 'Call us to book'. The single largest source of drop-off between intent and appointment.
• Ignoring why they left. A patient who had a bad experience needs that acknowledged before an offer.
• Measuring sends and opens. Neither pays for a hygienist's day.
• Setting recall intervals in the campaign. Interval is clinical; the campaign only carries the invitation.
