Common Pitfalls

Pitfall 1: Everyone is Accountable

Fix: Shared accountability = no accountability. One person is A, even if uncomfortable.

Pitfall 2: Too Many Cs

Fix: Over-consulting creates Decision by Committee. Only C people whose input changes the decision.
