Domain Deep-Dives — Rehearsing the Depth You Actually Have

Domain-technical interview rounds are where the most ambitious clinical-physician candidates make their biggest mistakes. The temptation is to prepare across breadth — refresh every therapeutic area, every regulatory pathway, every trial design archetype. The senior interviewer is testing something else entirely: can you reason under uncertainty, can you name what you don't know, can you build a credible argument from incomplete information? The right preparation goes deep on the user's actual expertise and rehearses the thinking pattern, not the encyclopedia.

Key Concepts

The 3-Depth-Areas Strategy

Before the round, identify the 2–3 areas you will go deep in:

1. Area 1: Your strongest domain. Your trial-readout area, your therapeutic area, your specific regulatory experience. Be ready to go 30 minutes deep here. This is where the interviewer will probe hardest if you signal mastery early.

2. Area 2: An adjacent domain you can speak to credibly. Not as deep as Area 1 but you can hold a substantive conversation. Often a methods area (biomarker enrichment, adaptive design) or a regulatory area.

3. Area 3: A current-events domain. A recent FDA decision, a public biotech readout, a debated topic in the field. Demonstrates that you're plugged into the conversation.

Everything outside these three areas, you signal honestly: "I don't have deep operational experience in [X], but here's how I'd think about it."

The Reasoning-Under-Uncertainty Pattern

When asked a question you don't fully know the answer to (which will happen), the senior pattern is:

1. Name what you know. "From what I know about [adjacent area], the relevant frameworks are X and Y."
2. Name what you don't know. "I don't have direct experience with [specific gap], so I'm reasoning by analogy."
3. Reason explicitly. "If the dynamics are similar to [analogous case], I'd expect [hypothesis]; if they differ in [way], I'd expect [alternative]."
4. Name what would resolve the uncertainty. "I'd want to look at [specific data], talk to [specific kind of expert], or run [specific analysis] to confirm."

This pattern — what philosophers call "structured uncertainty" — is what differentiates senior thinking from junior thinking. Most interviewers value it more than confident wrong answers.

Common Probe Types

What to Brush Up Before the Round

For an MD → biotech VC interview, a 1-week prep arc:

• Day 1: Review your own work — every trial you'd discuss, every decision, every result. The interviewer's depth probes will target your work; you must be ready for them.
• Day 2–3: Refresh your Area 1 domain — recent literature, recent regulatory decisions, current debates.
• Day 4: Refresh Area 2 — broad framework, key examples.
• Day 5: Read the firm's recent investments and any partner essays — Area 3 is current-events and firm-specific.
• Day 6–7: Rehearse with someone who can probe you. The probing rehearsal is the single most-leverage preparation.

Common Failure Modes

Self-Coaching Track

For your situation (MD → biotech VC/operator):

1. Pick your 2–3 depth areas. For a typical biotech VC interview, candidates: (a) your specific therapeutic area / trial type, (b) clinical-development methods broadly (endpoints, adaptive designs, biomarkers), (c) a current-events area aligned to the firm's thesis.

2. Build the "own work" prep doc. For every trial you'll discuss, write: design, sample size, endpoints, key amendments, results, regulatory interactions, what you decided, what you'd do differently. Be ready for any depth probe on this.

3. Hand off to Asclepius for depth content. The biotech-venture suite — Asclepius, clinical-development director, endpoint-selection, trial-design-optimizer — is the source of substantive depth content. Use it.

4. Build your field thesis. Before the round, articulate 2–3 specific views about where your field is going. Write them down. You will be asked.

5. Practice the structured-uncertainty pattern. Have someone ask you 5 questions at the edge of your knowledge. Practice naming-what-you-know, naming-what-you-don't, reasoning by analogy, naming what would resolve the uncertainty. Until it feels natural.

6. Mock with a real expert. A 1-hour mock with a friend who is actually senior in the area is worth 10 hours of solo prep. Ask them to probe hard.

Teach / Mentor-Others Track

When coaching a junior or peer through domain-deepdive prep:

1. Reframe from breadth to depth. Most mentees overdo breadth prep. Coach them to pick 2–3 areas and go deep. Use the 3-depth-areas framework.

2. The structured-uncertainty pattern is the highest-leverage idea. It transfers across every domain. Once a mentee internalizes it, they handle out-of-depth questions gracefully.

3. Force the "own work" prep doc. Many mentees prep by reading external content and forget that the most-probed area is their own work. Coach them to start prep with their own work, not external reading.

4. Field thesis is non-negotiable. Most mentees enter interviews without a clear view of where the field is going. They will be asked. Coach them to build a thesis before the round, even if tentative.

5. Mock with an expert is irreplaceable. Coach the mentee to find a real expert to mock with — not a friend, not a peer. The probing must be at the senior level.

6. Debriefing post-loop teaches faster than pre-loop prep. After each interview, walk through what was asked, where the mentee struggled, what they wish they had known. The debrief is where most of the learning happens.
