The biotech VC interview loop tests a small number of stable elements: do you have a thesis, can you do diligence, can you defend a portfolio decision, why do you want this job, and how will you behave in an IC room. For a clinical-physician candidate, every one of these is winnable — the underlying judgment is exactly the kind VC partners want — but only with deliberate preparation that translates clinical thinking into investment language. The candidates who lose are the ones who arrive with great clinical credentials and a vague sense of "I want to be in venture."

Beneath the formats, VC interviewers test:

1. Pattern recognition — Can you generalize from your work to other situations?
2. Judgment under uncertainty — Faced with incomplete information, do you make crisp decisions?
3. Thesis depth — Do you have views, and can you defend them?
4. Operator empathy — Will founders want you on their cap table?
5. Diligence rigor — Can you actually do the work?
6. IC presence — Will you contribute to the partnership conversation?
7. Want-this-job intensity — Is this the job for you, or one of several?

The MD-Operator Translation Challenge

VC partners pattern-match new candidates rapidly. The dominant patterns they recognize: consulting (clean structure, framework-heavy, sometimes thin on substance), finance (numbers-strong, sometimes thin on science), prior-VC (already speaks the language), founder (operator credibility, sometimes thin on diligence rigor). The MD-operator pattern is less common and therefore requires explicit translation. The user's job in the loop is to make the pattern legible: "I am bringing physician judgment to the work you do, here is what that means in practice, here are the specific diligence questions I'd ask differently."

The "Why VC" Answer

This is the single most-asked question in VC interviews and the single most-failed. The failure modes:

• "I want to learn about investing" → low intensity; junior framing
• "I want to help startups" → vague; doesn't address why VC specifically
• "I love biotech and want to back great companies" → generic
• "I burned out on the operator track" → flight, not pull

The good answer surfaces three things: (1) why this job is the optimal next move for your trajectory, (2) why now specifically, (3) what you bring that's structurally distinctive. The user's MD-operator edge is in component 3 — physician judgment in a domain where the dominant background is consulting/finance.

The Deal Review Answer Structure

When asked to evaluate an asset or company:

1. One-sentence thesis (would I invest? at what stage?)
2. The 2–3 things that would make this a yes
3. The 2–3 things that would make this a no
4. The diligence I'd run if I had 4 weeks
5. The kill question — what would I want to verify above all else

The Portfolio Review Preparation

Before any VC interview, review at least 3–5 of the firm's recent investments. For each: what's the thesis, what's the asset/program, where are they in development, what's the risk profile. Be ready to discuss any of them. The interviewer will sometimes ask "which of our portfolio is most interesting to you and why" — a question that punishes generic preparation.

2. Recent deals (12–18 months)
3. The partner you'd report to — their writing, talks, deals they led
4. The fund's stage and check-size range

Self-Coaching Track

For your situation (MD → biotech VC):

1. Build your "why VC" answer. Three components: why this move now, why VC vs operator, what you structurally bring. Draft it. Refine until it lands in 90 seconds with energy.

2. Build your field thesis. One page: 3 trends you believe in, 1–2 contrarian views, what you'd back at this stage of the cycle. Be specific about indications, modalities, stage.

3. Build a sample diligence. Pick a recently-public biotech (or one of the firm's portfolio companies). Run a structured diligence using Asclepius. Have a write-up ready to discuss — it's an artifact you can reference and demonstrates pattern.

4. Prepare 3 portfolio-company answers. For the firm's portfolio, prepare 3 companies you can discuss in depth: why interesting, what risks you'd flag, how you'd help if you were on the cap table. Pick companies in your clinical area.

5. Prepare for the case. Many firms send a deck or one-pager for a take-home or live exercise. Practice on past pitch decks; produce a 1-page diligence memo in 90 minutes. This is the muscle.

6. Rehearse with a real VC mentor. A real partner or principal will probe in ways you can't anticipate alone. Single most-leverage prep activity.

7. Reference round audit. Think about who in your network this firm might call. Make sure they're warmed up — a strong reference can move a borderline decision; a tepid reference can kill one.

Teach / Mentor-Others Track

When coaching a junior or peer through VC interview prep:

1. Diagnose "why VC" first. Most mentees have a weak why-VC answer. Until that's strong, no other prep matters; the first conversation will end the loop.

2. Force the field thesis exercise. Many mentees enter loops without an articulated thesis. They will be asked. Coach the production of a thesis doc before any interview is scheduled.

3. The portfolio review prep is non-negotiable. Mentees often skip this; they get burned in the loop. Insist on at least 5 portfolio companies prepared.

4. The sample diligence artifact is high leverage. Even if the firm doesn't ask for one, having an artifact you can reference ("I've been working through this on a public biotech, [Company X]") demonstrates pattern.

5. Translation is the hardest skill. Many physician mentees recite clinical work in clinical vocabulary. Coach explicit translation: "in your operator language, what does this mean to me as an investor?" Use the credibility-translation matrix.

6. The IC simulation is where senior loops are won or lost. Coach the structure (one-sentence thesis, yes/no factors, diligence plan, kill question). Practice live cases until the structure becomes reflex.
