Overview
The WHO Surgical Safety Checklist is a digital implementation of the World Health Organization's evidence-based surgical safety protocol. Introduced in 2008 as part of the WHO "Safe Surgery Saves Lives" initiative, this three-phase checklist has been demonstrated to reduce surgical mortality by 47% and major complications by 36% across diverse hospital settings worldwide (Haynes et al., NEJM 2009).
Clinical Context
An estimated 310 million surgical procedures are performed worldwide each year, with major complication rates of 3-17% and death rates of 0.4-0.8% in developed countries. Many surgical complications are preventable through systematic verification of critical safety steps. The WHO Surgical Safety Checklist addresses the most common categories of surgical error: wrong patient/site/procedure, anesthesia equipment failure, retained foreign bodies, and communication breakdowns.
Three Phases
Sign In (Before Anesthesia Induction)
Verifies patient identity, procedure, consent, and surgical site marking. Assesses anesthesia risks including difficult airway, allergy history, and anticipated blood loss. This is the last reliable opportunity to catch identification errors.
Time Out (Before Skin Incision)
Full team introduction and verbal confirmation of patient, procedure, and site. Surgeon briefs on critical steps and anticipated duration. Anesthesiologist reports patient-specific concerns. Nursing confirms sterility and equipment readiness. Antibiotic prophylaxis timing is verified.
Sign Out (Before Patient Leaves OR)
Procedure documentation verified. Instrument, sponge, and needle counts confirmed. Specimens labeled. Equipment issues documented. Key recovery and management concerns communicated to the entire team.
