Permit a permanent code change only after evidence distinguishes the proposed mechanism from credible alternatives. Report a fix only after replaying the original failure under comparable conditions.
Preserve boundaries and evidence
- Read applicable repository instructions first. Record

1. Establish the bug contract
Inspect the repository, tests, CI configuration, history, runtime output, and environment before asking questions. Establish:
- expected behavior and the independent source that defines it;
- actual behavior and exact failure signature;
- impact, affected scope, and whether the defect is confirmed or only reported;
- environment, configuration, dependency state, input state, and reproduction frequency;
- last known good, first known bad, and relevant recent changes when available;
- existing test seams, repository commands, data sensitivity, and authorization limits.
Ask only for material facts that cannot be discovered locally. Do not convert a vague report into a rigid questionnaire before inspecting available evidence.
2. Select the operating mode
- Triage: confirm an incomplete but explicit investigation request, distinguish product, test, fixture, environment, and expected behavior, then decide whether a concrete defect exists.
- Full diagnosis and repair: use by default for a concrete local or CI defect. Continue through a minimal repair after the causal checkpoint.
- Diagnosis only: remain read-only when the user asks why, requests a root cause without modification, or sets a read-only boundary.
- Observational diagnosis: use existing CI output, telemetry, traces, or dumps when the failure cannot be replayed locally. State confidence and do not claim

3. Establish an authentic failure signal
Name one repeatable operation or bounded observation that exposes the reported symptom. It may be a test, command, request, browser workflow, trace replay, benchmark, differential comparison, bisection predicate, or structured manual capture.
Accept the signal only when it is:
- authentic: reaches the relevant behavior rather than a guessed imitation;
- specific: distinguishes the reported failure from unrelated setup failures;
- red/green capable: can detect both the broken and repaired states;
- repeatable or measured: deterministic, or expressed as a failure rate under controlled conditions;
- safe and restorable: avoids unauthorized side effects and can reset changed state;
- practical: focused enough to run repeatedly with the available environment.
Run it before forming a causal conclusion and preserve the compact redacted result. When local replay is unavailable, define the exact observational signature and the evidence that would disconfirm the current interpretation.
If no authentic signal can be established after reasonable safe approaches, stop speculative repair. Report attempted approaches, observations, ruled-out explanations, best remaining hypotheses, and the exact missing capture, environment, or permission that would distinguish them.
4. Reduce and localize
Reduce inputs, steps, state, configuration, and dependencies one controlled factor at a time when reduction improves the investigation. Re-run the signal after each change and record what remains load-bearing. Do not minimize away the interaction that makes the failure authentic.
Localize where correct behavior first diverges:
- Compare working and failing inputs, environments, versions, or configurations under the same operation.
- Check inputs, outputs, state, and configuration at meaningful boundaries.
- Follow incorrect state backward toward its origin rather than patching where it becomes visible.
- Bisect code, test order, configuration, dependencies, datasets, or component boundaries when a reliable predicate exists.
- Determine whether the defect belongs to product code, test code, fixture/data, tooling, environment, or an external contract.
5. Run discriminating experiments
Maintain separate records for observations, inferences, and hypotheses. Preserve negative evidence so failed ideas are not silently repeated.
When ambiguity warrants alternatives, keep a short ranked set. For the next hypothesis state:
- the proposed causal mechanism;
- which observations support it;
- the predicted result if it is true;
- the result that would weaken or refute it;
- the cheapest safe experiment that changes one meaningful variable;
- risk, reversibility, cleanup, and confidence.
Choose experiments by information gain, plausibility, safety, reversibility, and cost. Add only probes that distinguish live hypotheses. Prefer a debugger, trace, targeted assertion, or boundary measurement over broad logging. Mark temporary instrumentation with a unique task-local marker and remove it before completion.
Do not narrate progress as proof. Reopen assumptions and alternatives when evidence conflicts with the current model. Never claim that all possible causes have been eliminated unless the bounded hypothesis space and evidence actually justify it.
6. Cross the causal checkpoint
Before permanent edits, state:
- the exact symptom and authentic reproduction or observational signature;
- the localized component, boundary, or state transition;
- the causal chain from triggering condition to first incorrect state to visible symptom;
- supporting evidence, remaining alternatives, and confidence;
- the minimal proposed repair and why it acts at the source;
- the correct regression seam or why no trustworthy seam exists;
- expected side effects, rollback considerations, and verification plan.
For an ordinary authorized local repair, this checkpoint is informational and does not require a reply. Stop for user direction when the patch requires new authority, a material architecture decision, dependency changes, destructive state changes, or uncertain high-impact behavior.
7. Repair with discipline
- Prefer a regression guard in the repository's existing test infrastructure at the lowest stable seam that still exercises the real bug pattern.
- Demonstrate red-before and green-after when feasible. If no valid seam exists, document that constraint instead of adding a shallow test that creates false confidence.
- Make one causally coherent repair. Avoid unrelated cleanup, opportunistic refactoring, broad catches, silent fallbacks, weakened assertions, deleted tests, and retry-based masking.
- Coordinate with

8. Verify and restore
Verify each applicable layer independently:
- the regression guard detects the unfixed behavior and passes after repair;
- the original unminimized scenario now succeeds under comparable conditions;
- relevant boundary and neighboring cases still work;
- affected subsystem tests and repository checks pass;
- flake, concurrency, performance, compatibility, or environment checks meet their specialized criteria;
- the final diff contains no unrelated changes, unexplained generated files, accidental dependency drift, sensitive artifacts, or temporary probes;
- task-started services, files, state, and environment changes are cleaned up or explicitly retained.
Do not turn one green run into proof for an intermittent or performance defect. Report checks that could not run and distinguish direct verification from inference.
9. Report the outcome
Return exactly one overall outcome:
FIXED: a causal repair was completed and the original signal plus relevant validation now pass.
DIAGNOSED: the causal mechanism is established, but repair was not requested or cannot be performed safely.
INCONCLUSIVE: safe evidence was gathered and narrowed or tested the issue, but it does not establish a trustworthy causal mechanism. The next discriminating observation may still be unavailable.
BLOCKED: meaningful investigation or repair cannot start or continue because required access, environment, permission, authority, or safe evidence is unavailable. Do not use BLOCKED merely because an otherwise useful investigation ended without a causal conclusion.
Report the bug contract, evidence and causal explanation, repair and regression guard, verification, repository hygiene, unresolved alternatives, residual risk, and precise next step. A conclusion is bounded by demonstrated evidence; never call it definitive beyond that boundary.
