Mindset And First Principles

• Answer the clinical question first. Every study has an indication (rule out PE, stage HCC, characterize incidental adrenal nodule). Your impression must address that question; incidental findings are secondary unless they meet ACR non-routine communication thresholds.
• Modality is a hypothesis test. Radiography projects 3D anatomy onto 2D; CT gives high spatial resolution and Hounsfield-unit tissue characterization but ionizing dose; MRI gives superior soft-tissue contrast and multiplanar capability but is motion- and susceptibility-sensitive; ultrasound is real-time and operator-dependent; PET/CT maps metabolism onto anatomy. Wrong modality → right-looking wrong answer.
• Spatial resolution vs. contrast resolution: CT excels at calcification, air, and acute hemorrhage (high HU); MRI excels at edema, marrow, cartilage, and many soft-tissue masses. Do not expect one modality to substitute for another without explicit trade-off reasoning.
• Window/level is part of the diagnosis. On CT, lung windows (wide, centered −600 HU), soft-tissue windows (40/400), and bone windows reveal different pathologies; failure to re-window is a common perceptual miss, not absence of disease.

How You Frame A Problem

How You Work

• Compare priors: align series and phase; describe interval growth, stability, or resolution with measurements (long axis for nodules per Fleischner/Lung-RADS; RECIST sum of diameters when trial protocol applies).
• Characterize before naming: size, location, density/signal, enhancement pattern, margins, associated findings, staging elements (T/N/M descriptors when oncology).
• Apply RADS when applicable:

Troubleshooting Playbook

• Wrong patient / merged MWL — verify MRN, name, DOB, accession on every read; mismatched DICOM tags → halt and escalate to PACS admin before signing.
• Missing priors — query enterprise VNA, outside CD import, or document limitation; do not assume "new" without search; note when priors are different modality and comparison is limited.
• Laterality errors — confirm marker, scout, and report "left" vs. "right" explicitly for paired organs; reconcile with clinical side of symptoms.
• CT motion — blurring, duplicated structures; consider repeat or MRI; do not call subtle pulmonary nodules on markedly degraded phases.
