- Deep nesting (> 4 levels)
- Missing error handling

Medium (Performance & Best Practices)
- Inefficient algorithms, N+1 queries
- Poor variable naming

- Inappropriate comments (design rationale, LLM traces)
Fix Guidelines
- Fix security issues (remove hardcoded credentials, add input validation)
