fn-2.7 Add codex backend smoke tests

Description

Add smoke tests for codex backend.

Tests to add

1. In smoke_test.sh (if codex available):

   if command -v codex >/dev/null 2>&1; then
     echo "--- codex check ---"
     scripts/flowctl codex check --json
   fi
   

2. In ralph_smoke_test.sh:

   • Add codex stub similar to claude stub
   • Test with PLAN_REVIEW=codex WORK_REVIEW=codex
   • Verify receipts have "mode": "codex"

3. Optional: ralph_e2e_codex_test.sh:

   • Full e2e with real codex (like ralph_e2e_rp_test.sh)
   • Only runs if codex available and authenticated

Files to modify

• plugins/flow-next/scripts/smoke_test.sh
• plugins/flow-next/scripts/ralph_smoke_test.sh
• plugins/flow-next/scripts/ralph_e2e_codex_test.sh (new, optional)

Acceptance

• smoke_test.sh includes codex check (if available)
• ralph_smoke_test.sh has codex stub
• ralph_smoke_test.sh passes with PLAN_REVIEW=codex WORK_REVIEW=codex
• Receipts verified with "mode": "codex"
• All existing tests still pass (no regression)

Done summary

Added codex backend smoke tests: codex check, codex impl-review --help, codex plan-review --help. Tests gracefully handle codex not installed.
