Implemented a production-ready evaluation harness for the skene-cookbook skills library, providing runtime validation, distributed tracing, metrics collection, and intelligent decision-making for AI skill execution.

Key Design Decisions
1. Non-Breaking Integration
Decision: Wrap existing skill execution rather than modify skill implementations.
Rationale:
- No changes needed to 808+ existing skills
- Skills remain implementation-agnostic
- Can be incrementally adopted

2. Multi-Factor Decision Logic
Decision: Use confidence × risk × validation for decisions.
Rationale:
- Single-factor (e.g., just risk) is too simplistic
- Combines multiple signals for better accuracy
- Configurable thresholds for different environments

Validation Failed → BLOCK
Critical Risk → REQUIRE_APPROVAL
Confidence < 0.30 → BLOCK
High Risk + Low Confidence → FLAG_FOR_REVIEW
Confidence ≥ 0.85 → AUTO_ACT
Confidence ≥ 0.50 → FLAG_FOR_REVIEW
Confidence < 0.50 → REQUIRE_APPROVAL
3. Hierarchical Tracing
Decision: Use span stack for automatic parent-child relationships.
Rationale:
- Simplifies nested tracing (no manual parent passing)
- Automatically captures workflow → step → skill hierarchy
- Compatible with OpenTelemetry standard

4. Separate Validation and Execution
Decision: Validate before and after execution, not during.
Rationale:
- Clear separation of concerns

5. Schema-First Approach
Decision: Load schemas from existing skill.json and metadata.yaml.
Rationale:
- Schemas already exist (inputSchema, outputSchema)
- No duplication of schema definitions
- Single source of truth
