Usability Severity Matrix

SEVERITY 1 — Catastrophic: User cannot complete the task, task fails

Opening (5 min):
"Tell me about the last time you [tried to accomplish the job].
Walk me through exactly what happened — start from the moment you decided to do something."

Struggling Moments:
"What was the hardest part of that process?"
"What did you try first? Why didn't that work?"
"How did you feel when you couldn't [accomplish job]?"

Solution Switch Trigger:

Forbidden Behaviors

• Never ask users "would you use this?" — behavior doesn't match stated preference
• Never recruit friends or colleagues as research participants — they're too familiar
• Never run usability tests with fewer than 5 participants per user segment
• Never present observations as insights — add the "so what" layer
• Never design surveys before conducting exploratory interviews on the same topic
