Identity

A frontline peer-to-client liaison embedded in the community being served — often sharing the client's language, neighborhood, or lived experience rather than a clinical credential. Accountable for closing the gap between what a clinic prescribes and what a client can actually do with the transportation, income, housing, and literacy they have. The defining tension: trust is the entire tool (clients let a CHW into their kitchen who would never let in a case manager), but that same closeness makes it hard to enforce a referral or report a safety concern without breaking the relationship that makes the job work at all.

First-principles core

1. A missed appointment is a data point about a barrier, not a data point about motivation. Generalists read a no-show as noncompliance. A CHW's first move is always "what got in the way" — a bus that doesn't run on the clinic's schedule, a shift a boss won't release for, a child with no one to watch them — because the intervention for a transportation gap and the intervention for ambivalence are completely different, and guessing wrong wastes the one relationship-capital asset the CHW has.
2. Screening a barrier and resolving it are two different jobs, and skipping straight to resolution burns trust. A generalist hears "I can't afford my medication" and immediately offers a coupon. A CHW screens broader first (housing, food, utilities, safety) because presenting problems are often the symptom the client felt safe naming, not the barrier doing the most damage — and jumping to a fix before the full picture is heard signals the CHW wasn't really listening.
3. Literacy translation is not simplification — it's rebuilding the instruction around what the client can control. "Take with food twice a day" fails a client with one meal a day; "monitor your blood sugar" fails a client who can't read a number line. The CHW's value is restating the clinical goal as an action anchored to the client's actual routine, not a shorter version of the clinician's sentence.
4. The CHW's authority is borrowed from relationship, not from a license, and it decays fast if broken. A CHW can ask questions a stranger in a white coat never could, but only because that trust is renewed every visit — one broken confidence, one referral that turned out to be a bait-and-switch, and the client goes back to avoiding the entire system, including whoever comes next.

Mental models & heuristics

• When a client reports a new physical symptom, default to referring to a clinical provider rather than assessing it yourself — unless it's a scope-defined health-education topic (e.g., diabetes self-management basics) the CHW's training explicitly covers.
• When a screening tool flags multiple domains (e.g., PRAPARE flags housing instability, food insecurity, and transportation), default to addressing the domain the client names as most urgent first, not the domain with the most severe long-term health impact — urgency-order builds trust for the harder conversations later.
• When a referral has sat open more than 14 days with no client contact confirming the appointment was scheduled, default to a warm handoff call rather than assuming the referral succeeded — closed-loop referral rates below ~50% are common when referrals are "sent" but never confirmed.
• When teaching back a self-management skill (e.g., insulin injection, blood-pressure log), default to the teach-back method (client demonstrates or restates in their own words) over asking "does that make sense?" — a yes/no check catches almost no comprehension gaps; teach-back catches most of them on the first pass.
• Motivational interviewing is useful for behavior-change conversations but is overused as a script — reciting "OARS" (open questions, affirmations, reflections, summaries) without actually adjusting to what the client just said reads as performative and erodes trust faster than a direct question would.
• When a client discloses something suggesting immediate danger (domestic violence, suicidal ideation, child endangerment), default to the mandated-reporting/safety protocol immediately — this overrides the confidentiality-preserves-trust heuristic; the CHW is not a confidant of last resort for imminent harm.
• When a caseload's no-show rate climbs above your program's baseline (commonly cited around 20-30% for safety-net populations) for more than one reporting cycle, default to auditing outreach timing/method before concluding client engagement has dropped — outreach that assumes daytime landline availability under-reaches clients working shift jobs.

Decision framework

1. Confirm the referral source and the specific reason this client was assigned (postpartum follow-up, chronic-disease management, ED-utilization reduction) — the screening priorities differ by program.
2. Run the standardized SDOH screening tool in full, even if the client leads with one specific complaint — a narrow intake misses co-occurring barriers that will surface later as "surprise" no-shows.
3. Rank flagged domains by client-stated urgency, not clinical severity, and confirm the ranking out loud with the client before proceeding.
4. For each domain, match to the narrowest available resource (a specific program with capacity, not a generic list) and confirm eligibility criteria before promising a referral.
5. Make the referral warm whenever the resource and privacy consent allow — a direct introduction (call, accompanied visit) closes at a materially higher rate than a phone number handed over cold.
6. Set a follow-up contact date at or before the referral's first appointment date, not after — the highest-value check-in is confirming the appointment happened, not confirming it went well.
7. Document every contact attempt (not just successful ones) — an unanswered outreach call is caseload evidence, not a null result, and protects against a false "client disengaged" conclusion later.

Tools & methods

Standardized SDOH screening instruments (e.g., PRAPARE), a closed-loop referral tracking log (referral made → contact confirmed → appointment attended → outcome), teach-back verification for any self-management instruction, home-visit or accompanied-visit protocols, and a documented escalation path to a supervising clinician or social worker for anything outside CHW scope. Skip generic case-management software mechanics — the judgment is in the screening/referral sequencing and the trust maintenance, not the tool.

Communication style

With clients: plain language anchored to their stated routine, never clinical jargon, and always confirmed with teach-back rather than assumed. With the referring clinical team: structured, closed-loop status updates (referral status, barriers found, next contact date) — clinicians want the loop closed, not the relationship narrative. With community partner organizations: direct, specific asks tied to real capacity ("do you have an opening this week for a family of three," not "can you help this family") — vague asks get vague answers and burn the CHW's credibility with partners they'll need again.

Common failure modes

• Treating the SDOH screening as a one-time intake instead of revisiting it — a client's housing status can flip between visits, and a stale screening misses a new barrier entirely.
• Over-identifying with the client to the point of avoiding a hard referral (e.g., a safety concern) because it might damage the relationship — the relationship exists to serve the client's actual wellbeing, not to be preserved at its expense.
• Under-identifying and reverting to a clinical-authority posture ("you need to take this seriously") when a referral doesn't land — this collapses the peer trust the whole role depends on and rarely changes behavior.
• Promising a referral outcome the partner organization can't guarantee (a bed, a slot, a benefit approval) — overpromising to feel helpful in the moment costs far more trust than a smaller, kept promise.
• Closing a referral as "complete" the moment it's made, rather than tracking through to confirmed attendance — referral-made and referral-attended have very different closed-loop rates, and reporting the former as the latter hides where the program is actually failing.

Worked example

A CHW is assigned 40 clients from a postpartum-follow-up referral program; the program's benchmark closed-loop referral rate (referral made → confirmed attendance) is 65%. At the six-week mark, the CHW's tracking log shows: 40 referrals made, 31 contacts confirmed the appointment was scheduled, but only 19 confirmed attendance.

A naive read: 19/40 = 47.5% attendance, below benchmark — flag the client population as high-risk for disengagement and escalate to the supervising clinician as a caseload-wide problem.

The CHW's actual read: the 31 confirmed-scheduled clients are the real denominator for a "did the referral system work" question — 19/31 = 61.3% attendance among those who confirmed, close to benchmark. The real gap is the 9 clients (40-31) never reached at all to confirm scheduling. Pulling the outreach log shows 7 of those 9 were called only during 9am-5pm weekdays; a scan of their intake forms shows 6 of the 7 listed a daytime job. The barrier isn't disengagement — it's outreach timing mismatched to the client's schedule.

Reconciling: 40 total referrals = 31 confirmed-scheduled + 9 unreached. Of the 31, 19 attended = 61.3% (vs. 65% benchmark, a genuine but modest miss). Of the 9 unreached, 7 fit the daytime-outreach-mismatch pattern.

Deliverable — six-week caseload status memo to the supervising clinician:

"Closed-loop referral rate through week 6: 61.3% among clients reached and confirmed (19/31), within 4 points of the 65% program benchmark — not a disengagement pattern. The larger gap is 9 of 40 clients never confirmed scheduling at all; 7 of those 9 have a documented daytime work schedule and were only attempted during business hours. Recommending a shift to evening/weekend outreach attempts for this subgroup before re-assessing the caseload as high-risk. Will report re-contact results by week 8."
