2. A quality measure's numerator, denominator, and exclusion logic is defined precisely by the measure steward, and a slight misinterpretation changes the reported rate materially. The wrong age range, an incorrect lookback period, or a missed exclusion criterion doesn't produce a slightly-off number — it can shift a reported rate by several percentage points, with real consequences for quality reporting, reimbursement, or public performance comparison.
3. A clinical decision support alert override rate above roughly 90% (well-documented across the literature for many alert types) signals alert fatigue, not clinician non-compliance, and the fix is redesigning the alert, not retraining clinicians. An alert firing on overly broad criteria trains clinicians to dismiss it reflexively — treating a high override rate as a compliance problem rather than a specificity problem keeps a genuinely dangerous situation (an alert nobody reads anymore) in place.
4. An interoperability failure where data appears "missing" between systems is frequently a terminology or code-mapping problem, not an actual data gap. A hospital's internal lab test codes that aren't correctly mapped to a standard vocabulary (LOINC) will fail to transmit or match meaningfully in an interoperability exchange, even though the underlying clinical data exists and is complete in the source system — the fix is fixing the mapping, not concluding the data doesn't exist.
5. HL7 v2 and FHIR are structurally different data models, and assuming a straightforward field-to-field mapping between them risks silent data loss during a system integration. HL7 v2's pipe-delimited message segments and FHIR's RESTful JSON resources organize clinical information differently — mapping at the individual-field level without accounting for how each standard actually structures the underlying clinical concept can silently drop or misinterpret data during a migration or integration.

Mental models & heuristics

• When building or reviewing a quality measure report, default to checking whether the required data elements are captured as structured/coded fields, not free text, before trusting the calculated rate — a rate calculated only from structured data will silently miss results documented in unstructured form.

Worked example

An automated eCQM calculation reports a diabetes "Hemoglobin A1c control" measure rate for a health system's measurement year.

Measure specification (per steward): Denominator = patients aged 18-75 with a diabetes diagnosis and a qualifying encounter during the measurement year. Numerator = patients whose most recent A1c result during the measurement year is under 8%. Exclusions: specific comorbidity/frailty indicators, hospice patients.

Initial automated report: Denominator = 1,200 patients. Numerator (A1c under 8%, from structured LOINC-coded lab results) = 620 patients. Reported rate = 620 ÷ 1,200 = 51.7%.

Manual chart review validation (50-record sample): Of the sampled patients counted as not meeting the numerator, 8 of 50 (16%) actually had a qualifying A1c result under 8% documented during the measurement year — but it was captured only as a scanned PDF lab report attachment in the chart, not as a structured, LOINC-coded discrete lab result. The automated calculation, which only reads structured lab data, missed these results entirely.

Extrapolation to full population: The 16% discrepancy rate found in the sample is applied to the full non-numerator population.

• Non-numerator count: 1,200 − 620 = 580 patients.
• Estimated additional true-positive numerator patients: 580 × 16% = 92.8 ≈ 93 patients.
• Corrected estimated numerator: 620 + 93 = 713 patients.
• Corrected estimated rate: 713 ÷ 1,200 = 59.4% — a 7.7 percentage point understatement in the originally reported 51.7% rate, driven entirely by unstructured (scanned) lab data not being captured in discrete, coded fields.

Findings and recommendation memo:
