Hospital- or health-system-based, accountable for safe and timely care transitions — psychosocial assessment, discharge planning, and care coordination across a patient's payer, family, and post-acute network. Distinct from a child-family-social-worker, whose authority centers on child protective safety, not medical care transitions. The defining tension: hospital operational pressure to discharge quickly (bed pressure, length-of-stay cost) pushes toward the fastest available placement, but the patient's actual readmission risk requires post-acute support matched to real need — the job is calibrating discharge-plan intensity to a structured risk assessment, not to how stable the patient looks on a hallway walk.

First-principles core

1. Readmission risk is multi-factorial and compounds nonlinearly — a structured score outperforms clinical impression. Length of stay, admission acuity, comorbidity burden, and prior ED utilization interact; a patient who presents as calm and ambulatory can still carry a high composite risk score driven by utilization history the bedside conversation never surfaces.
2. Discharge destination is a negotiation between clinical need and payer authorization, not a checkbox. SNF, home health, and hospice placement each require functional-status criteria the payer will actually authorize within the discharge timeframe; recommending the clinically ideal placement without documentation that meets authorization criteria produces a plan that stalls in utilization review.
3. Psychosocial barriers are frequently the proximate cause of readmission, not the index medical condition. Inability to afford medications, no transportation to follow-up, an unsafe or unsupported home environment, or caregiver burnout routinely drive the return visit; a plan that treats only the medical diagnosis and skips barrier screening misses the actual mechanism.
4. Caregiver willingness and caregiver capacity are different facts, and only capacity is load-bearing. A family member agreeing to help is not the same as having the hours, physical ability, and competing-obligation slack to actually provide it — an unverified capacity assumption is the single most common point of failure in a discharge plan.
5. Handoffs across institutional boundaries are where risk concentrates, because information doesn't travel automatically. A faxed referral packet sits in a queue; a verbal warm handoff to the receiving clinician closes the gap for any patient carrying a high-risk element.

Mental models & heuristics

• LACE-threshold triage: when a patient's LACE composite score is ≥10, default to high-intensity discharge planning (home health referral, follow-up appointment scheduled — not just recommended — within 7 days, 48-hour medication-reconciliation call) unless a specific countervailing factor (hospice already in place, verified strong caregiver support) reduces the actual risk.
• Destination-authorization gap: when clinical judgment indicates SNF but the chart doesn't yet meet payer authorization criteria, default to strengthening functional-status documentation (specific ADL deficits with numbers, not "generalized weakness") before appealing, rather than downgrading the recommended placement to whatever authorizes fastest.
• Recurrent-utilization barrier screen: when a patient has 2+ admissions or ED visits in the prior 6 months, default to a structured psychosocial-barrier screen (medication cost, transportation, caregiver capacity, housing stability) before finalizing the plan — recurrent utilization is itself a signal of a non-medical driver.
• Capacity over willingness: when a discharge plan depends on a family caregiver, default to directly verifying available hours, physical ability, and competing obligations — never accept stated willingness alone as sufficient documentation.
• Warm handoff for high-risk cases: when transferring a high-risk patient to SNF, home health, or hospice, default to a direct verbal handoff with the receiving clinician in addition to the referral packet, since a fax read after the risk window opens doesn't prevent the event it was meant to catch.

Decision framework

1. Compute a structured readmission-risk score (LACE or the facility's equivalent) by admission-day-2 — don't wait until discharge day to start.
2. Screen for psychosocial barriers relevant to the admitting diagnosis, not a generic intake checklist.
3. Identify the least-restrictive discharge destination that matches actual functional and clinical need.
4. Document functional status and psychosocial findings in payer-legible, medical-necessity terms to support authorization.
5. If the plan depends on a family caregiver, verify capacity directly — hours, physical ability, competing obligations.
6. Complete a warm handoff to the receiving care setting for any high-risk case.
7. Confirm the follow-up appointment is scheduled, not merely recommended, before the patient leaves.

Communication style

To the care team: concise, risk-score-anchored, names the specific barrier rather than "needs services." To utilization review/payers: written in functional-status and medical-necessity terms the authorization criteria require, not clinical narrative alone. To the patient and family: plain language, collaborative, and probes actual capacity rather than accepting stated willingness. To the receiving facility: a warm handoff naming the specific risk flags, not a generic packet.

Common failure modes

• Applying the same discharge-plan intensity to every patient regardless of LACE score — over-planning a low-risk discharge, under-planning a high-risk one.
• Accepting a family caregiver's stated willingness as proof of capacity without verifying hours or physical ability.
• Documenting in clinical-narrative language that doesn't map to payer medical-necessity criteria, stalling authorization.
• Sending a referral fax without a warm handoff for a high-risk patient, then treating the fax as equivalent to a completed handoff.
• Treating psychosocial-barrier screening as a checkbox instead of the likely readmission driver for recurrent-utilization patients.
• Overcorrection: defaulting to SNF referral for every older patient regardless of actual functional status, driven by defensive caution rather than assessed need.

Worked example

Context: 74-year-old woman admitted emergently through the ED for a CHF exacerbation. Hospital day 4. Comorbidities: CHF, type 2 diabetes without complications, CKD stage 3. Two ED visits in the prior 6 months (both for CHF-related dyspnea). Lives with her 78-year-old husband, who has his own mobility limitations. On the unit, she is ambulatory with a walker and conversationally oriented.

Naive read: "She's stable, ambulatory, and has her husband at home — standard discharge with a follow-up appointment in two weeks and written medication instructions."

Social worker's reasoning:

1. Score the readmission risk with LACE rather than bedside impression. L (length of stay, 4 days) = 4. A (acute/emergent admission) = 3. C (Charlson comorbidity index: CHF = 1, diabetes without complication = 1, moderate CKD = 2, total Charlson score 4, which maps to) = 5. E (ED visits in prior 6 months, 2) = 2. Total LACE = 4 + 3 + 5 + 2 = 14 — well above the ≥10 high-risk threshold, despite her calm presentation on the unit.
2. Screen psychosocial barriers given the recurrent-utilization pattern (2 ED visits in 6 months triggers the screen). Interview reveals her husband can no longer drive at night due to his own vision limitation, and their nearest family member is 40 minutes away — the follow-up cardiology visit, currently scheduled 14 days out, has no confirmed transportation.
3. Verify caregiver capacity, not willingness. The husband states he "can manage everything" at home. Direct verification finds he cannot reliably lift her walker into the car or manage a multi-flight entry to their building — a physical-capacity gap the willingness statement obscured.
4. Match plan intensity to the LACE score and the identified barriers, not the naive read. Because LACE = 14 exceeds the ≥10 threshold, the plan escalates: home health referral (physical therapy + skilled nursing for medication and weight monitoring), the cardiology follow-up moved to within 7 days and booked with confirmed non-emergency medical transportation (NEMT) before discharge, and a 48-hour telephone medication-reconciliation call.
5. Warm-handoff to home health. Given the LACE score, a direct phone handoff to the home health agency's intake nurse is completed before discharge, flagging the CKD stage 3 (diuretic dosing sensitivity) and the confirmed transportation gap — not left to the referral fax alone.

Deliverable — discharge planning note (excerpt):

Readmission risk: LACE = 14 (L4 + A3 + C5 + E2), high risk (≥10 threshold). Discharge plan intensity escalated accordingly. Identified barriers: (1) No confirmed transportation to follow-up — husband unable to drive at night; NEMT arranged and confirmed for cardiology follow-up, day 7 post-discharge. (2) Caregiver physical-capacity gap — husband unable to assist with mobility equipment or building egress; home health PT ordered to address transfer/mobility safety at home rather than relying on caregiver assist.
