1. Safety is the actual first constraint, not a slogan, because the cost of getting it wrong is irreversible in a way every other production metric isn't. A production decision that trades safety margin for throughput is categorically different from a decision trading, say, cost against speed — the potential for injury or death changes the calculus in a way no amount of output gain offsets.
2. The bottleneck determines total system throughput, and optimizing anywhere else is largely wasted effort. A production line's output is capped by its slowest, most constrained step — improving a non-bottleneck station's speed just produces more work-in-progress inventory sitting in front of the real constraint, not more finished output.
3. Quality problems caught upstream are cheap; the same defect caught downstream (or by a customer) is expensive in a way that compounds with distance traveled through the process. A defect caught at the station that created it costs a rework; the same defect discovered after shipping costs a recall, brand damage, and the original rework anyway — quality control invested early is disproportionately more valuable than the same effort invested late.
4. Standard work makes deviation visible, and without a standard, every variation looks normal. A documented, consistent process for a given production step is what makes it possible to notice when something's actually going wrong — without a baseline, drift and inconsistency are invisible until they produce a quality failure or safety incident.
5. Workforce fatigue and morale are production inputs with measurable effects on quality and safety, not soft HR concerns separate from operations. Overtime and understaffing that look like they're maintaining output in the short term reliably show up as rising defect rates and incident rates with a lag — treating the workforce as infinitely elastic capacity is a real operational failure mode, not just a people-management one.

• Theory of constraints / bottleneck-first optimization: identify the single limiting step in the production process and focus improvement effort there — gains anywhere else in the line don't translate into more finished output until the actual constraint is addressed.

Decision framework

1. Any production decision gets checked against safety first, as a non-negotiable filter, before throughput, cost, or schedule considerations are weighed — a decision that meaningfully compromises safety margin doesn't proceed regardless of the output gain it offers.
2. Identify the actual bottleneck before investing improvement effort anywhere — map the process to find the true constraint, rather than intuitively optimizing the most visible or most recently problematic station.
3. Push quality control as far upstream as feasible — catching a defect at its point of origin is close to always cheaper than catching it later, so inspection and error-proofing investment should be weighted toward the earliest points in the process.
4. Establish and maintain standard work for critical processes before attempting to optimize them — without a baseline, it's not possible to reliably distinguish a real improvement from noise, or real drift from normal variation.
5. Monitor workforce fatigue/staffing indicators (overtime trends, unfilled shifts) as leading indicators, and treat a sustained trend as a signal to address staffing or scheduling, not just a cost to absorb as long as short-term output holds.

Common failure modes

• Trading safety margin for throughput under schedule pressure — treating a safety concern as negotiable against a production deadline, when the two are not actually comparable categories of tradeoff.
• Optimizing a non-bottleneck station — investing visible improvement effort on a station that isn't actually constraining total line output, producing activity without moving the real metric.
• Inspecting quality in at the end instead of building it in early — relying on final inspection to catch defects instead of investing in upstream error-proofing and in-process quality checks, incurring much higher cost per defect caught.
• Optimizing without a standard baseline — attempting continuous improvement on a process that was never documented as a consistent standard, making it impossible to reliably tell whether a change actually helped.
• Treating overtime as free capacity — leaning on sustained overtime to hit output targets without accounting for the lagged rise in defect and incident rates that chronic fatigue produces.
• Single-metric equipment evaluation — judging equipment or line performance by one dimension (speed) while missing a tradeoff on another (quality, uptime) that a composite metric like OEE would have caught.

Worked example

Situation: A 5-station line is missing its 1,200 units/day target, running 980 units/day (82% of target). The instinct is to speed up Station 5 (final assembly), the most visible and recently problematic step.

Step 1 — check station capacities before committing to a fix. Per-day capacity at each station (8-hour shift): Station 1 (prep) 1,400, Station 2 (subassembly) 1,280, Station 3 (core assembly) 976, Station 4 (QC/test) 1,240, Station 5 (final assembly) 1,120. Station 3 is the lowest-capacity step — the actual bottleneck.

Step 2 — confirm with WIP data, not just the capacity table. Work-in-progress inventory measured immediately upstream of Station 3: 340 units queued, growing +15 units/day — exactly the signature of a bottleneck (inventory piles up right before the true constraint). No comparable pileup exists in front of Station 5.

Step 3 — check that the bottleneck's capacity explains the actual output. Station 3's 976 units/day capacity almost exactly matches the observed 980 units/day actual output — confirming Station 3, not Station 5, is what's actually capping the line.

Step 4 — price the proposed fix (speeding up Station 5) against this finding. Adding overlap-shift labor at Station 5 would raise its capacity from 1,120 to 1,400/day at a cost of $340/day — but since Station 3 caps total output at 976/day regardless, this investment produces zero increase in finished output; it only shifts WIP buildup to a different point in the line. $340/day spent for no output gain.

Step 5 — price the fix that actually addresses the bottleneck. A second fixture at Station 3 (one-time capital cost $28,000) is projected to raise its capacity from 122/hr (976/day) to 150/hr (1,200/day) — closing the gap to the 1,200/day target exactly. At a $18/unit margin, the additional 220 units/day (1,200 − 980) generates $3,960/day in additional margin. Payback: $28,000 ÷ $3,960/day ≈ 7.1 days.

Deliverable (capacity investment recommendation, quoted):

Recommendation: fund the $28,000 second fixture at Station 3, not the $340/day overtime addition at Station 5. Station 3 is the confirmed bottleneck (976 units/day capacity, matching actual output almost exactly, with WIP piling up directly upstream of it) — Station 5 has never been the constraint. The Station 3 fixture raises capacity to 1,200/day, meeting the target, with a 7.1-day payback at current margins. Speeding up Station 5 as originally proposed would have cost $340/day in overtime for zero change in finished output.
